Caught in, under or between a moving and a stationary object

An employee using an ironworker sustained a finger injury. Taken to ER for treatment. Has been taken off work.

Struck by falling object

EE was in the process of closing an overhead ventilation door when the counter weight cable failed causing the door to fall and strike the left foot. EE was evaluated by a Physician and taken off of work.

Over-exertion in lifting objects

EE was re positioning a stop rail, as EE picked it up EE felt a pop in left wrist. Evaluated by MD on 1-16-2016 and put on restricted duty.

Struck by... (Not Elsewhere Classified)

A mobile jib crane was being used to lift a piece of grating. When the grating did not move the crane was readjusted while still attached and under pressure. The grating cross bar failed and caused the chain assembly and crane arm to strike the EE in the face. EE was treated by MD and taken off work.

Over-exertion in lifting objects

Employee had been moving 12"" x 12"" blocking with a partner. After finishing job employee was riding in a truck and began to feel pain in right inner thigh. Was seen by NP and returned to work. On 8-15-16 employee returned from one week of vacation and complained of increased pain. Seen by MD on that date and put on restricted duty.

EE was trying to open a food package with a knife. The knife went through the package and cut the EE's left index finger. The EE was taken to the hospital where EE received 4 stitches and returned back to work full duty.

Over-exertion (Not Elsewhere Classified)

Employee was walking down stairs and slipped. Had complaint of sore shoulder. The employee was evaluated by a physician on 2-4-16. Physical Therapy was ordered.

Over-exertion (Not Elsewhere Classified)

Employee complained of discomfort in right hand and was seen by a doctor and evaluated for repetitive trauma. It was made known on 6-2-16 that work activities were a contributing factor to this illness.
