Caught in, under or between a moving and a stationary object

EE and Another EE responded to a stuck pallet alarm on Unit IV cooler. He became trapped when the cooler pallet released on its own causing the dump arm and wheel to swing up and strike him in the head, pinning him between the dump arm wheel and the rail.

Struck by... (Not Elsewhere Classified)

Working on a pebble crusher hydraulic locking post system when he was struck on his arm by the quick disconnect valve fitting. He required surgical flushing and debridment of the site.

Over-exertion (Not Elsewhere Classified)

Working on cooler, replacing carrying rollers. This requires a considerable amount of kneeling in awkward positions. His knee started to ache at the end of the job and shift. Treatment was sought and he eventually required surgery on 12/09/05.

Working on replacing strippers employee took a stripper off and went to lay it on the grating. His finger was caught between and pinched. He received stitches and a fractured fingertip.

Fall to the walkway or working surface

While walking through the Weld Shop he tripped on a blown over welding screen. Strained right shoulder. He was treated and eventually ended with a surgical repair on 10/10/05.

Struck against stationary object

Employee was moving into position to clean jets in the siphonsizer when she stepped into the drain hole and cut her leg.

Fall down stairs

Slipped and fell down the stairway landing on his left shoulder

Over-exertion in pulling or pushing objects

While trying to bar liner into place the weight of the liner shifted onto the pry bar. Employees back was strained while trying to accomodate the shift of weight. He lost one day of work.

Rubbed or abraded

Employee was kneeling on walkway grating while repairing/replacing same and felt pain in knee at end of shift.

A wiper had pushed a belt up against the steel support arm and stopped the belt from running. The bracket that holds the wiper in place was removed. The tension on the wiper released and struck employee on the bridge of the nose. He required stitches.

Fall to the walkway or working surface

Climbing up a bank in the Pit when he fell hitting his wrist on a sharp rock. He received stitches.

Over-exertion (Not Elsewhere Classified)

While moving/rolling movable stairway in cold storage area, heard right knee pop to side and he rolled to the ground. He was treated and prescribed physical therapy on 12/15/05.

Struck against stationary object

Employee was carrying a part of a troughing frame on a galleyway that narrows. He hit his head on a unistrut, used to support a cable tray. He strained his neck. Physical therapy was ordered 6/28/05

While walking along the siphonsizer walkway he struck his head against a low structural beam and strained his neck. He was later prescribed Physical Therapy.

Struck by flying object

One employee tossed a can of cleaner across the room to another employee. The can cut the ear of another employee and it required stitches.

While removing nuts and bolts that hold the liners in place, the liner slipped off the wall and pinched his finger between the nut and the mill face. He required stitches and suffered a fractured fingertip

Employee was taking the mechanism off the old screen from line 17. After cutting the last huck bolt off, he took a drift pin to punch the bolt out. When he did this the mechanism dropped down & drove the drift pin against the screen pinching his hand between the screen and the drift pin. This resulted in 3 stitches.

Removing trommel liners with porta power the liner was pryed from the trommel frame. It pinched employee's finger between the frame and the liner. He required stitches to his finger.

Employee complained of soreness in right elbow due to work activities. He was diagnosed with epicondylitis and on 10/27/05 it was determined to be work related.

Employee complaining of soreness in both hands due to work activities. He was diagnosed with Carpal Tunnel Disease and on 10/27/05 it was determined to be work related.

Employee complaining of soreness in bilateral hands. He was diagnosed with Carpal Tunnel Disease and on 10/27/05 it was determined to be work related.

Employee with work-related bilateral carpal tunnel syndrome underwent surgery on right hand 6/23/14. Left hand surgery scheduled for 7/9/14. Employee is off of work at this time.
