The employee was removing an engine panel from a rubbertired front end loader when the panel slipped from his grasp. The employee tried to keep the panel from falling injuring his back.

EE WAS LIFTING A 60-70# BUCKET OF AGGREGATE TO DUMP IT INTO THE SAMPLE SPLITTER. BUCKET WAS SITTING ON THE FLOOR AND NEEDED TO BE DUMPED IN SPLITTER (~39"" HIGH). EE REPORTED THAT HE FELT SOM ETHING ""PULL"" IN HIS LOWER BACK OR RIGHT HIP WHILE ATTEMPTING TO LIFT. THIS BECAME REPORTABLE ON 9/22/09 AFTER EE WAS MOVED TO DIFFERENT POSITION DUE TO ADDITIONAL RESTRICTIONS.
Injured employee was using a come-a-long attached to a sheave on a 4 foot cone crusher. While he was pulling the come-a-long handle back and forth, the chain slipped and he lost his balance and fell back approximately 57"" to the ground.
RAINY, SLIPPERY & MUDDY. EE STEPPED DOWN OFF OF LADDER IN TIGHT QUARTERS ONTO UNEVEN FLOOR, TURNING ANKLE UNDER RIGHT LEG.
STEEL PLATE FELL ON INJURED EMPLOYEE'S ANKLE WHILE PERFORMING MAINTENANCE CAUSING FRACTURE.
Employee strained Left Arm while using sledge hammer
Cleaning the deck at head end of conveyor 2. He was picking up a rock to put on conveyor 3 when another rock fell off from conveyor 2 and struck his right pinky finger and smashed it between 2 rocks.
EE assisting foreman with changing screen cloth on Wash Screen S-2. While installing middle deck screen, the employee bent down through the first deck with arms to bolt down center rail hitting back of left arm on worn angle iron, causing cut to back of arm. Resulting in three stitches.
Employee was carrying the a grease gun down the steps at the primary crusher. As he descended, he slipped and fell backwards, striking his elbow on one of the steps. The resulting impact lacerated Elbert's elbow. Upon investigation of the site the stairway was free of debris and did not appear to be slick.
Employee was climbing into crane when he felt a strain in his left shoulder.
While installing strut bearing on truck, a piece of brass entered his eye behind his safety glasses. He was using a brass hammer.
Employee was carrying a bundle of conduit and walking beside a 36"" deep trenchline. Employee lost his footing and slipped into the trenchline, causing the employee to sprain his left knee. The employee has not lost any time due to the injury.
EE WAS TIGHTENING THREADED DOOR LATCHES ON EAST ACCESS DOOR OF WEST HAZEMAG CRUSHER USING AN 1 1/2"" COMBINATION WRENCH. IN ORDER TO TIGHTEN TOP LATCH EE USED A HAMMER TO STRIKE THE WRENCH. WRENCH SLIPPED OFF THE HEX NUT ON LATCH PINCHING THE EE'S FINGER BETWEEN THE WRENCH AND LOWER LATCH.
Victim scaled fence adjacent to public roadway and jumped to quarry floor below.
