Accident type, without injuries

The operator is filing this 7000-1 under protest and to comply with the mandatory abatement provisions of the act. Constructing a pond, personnel filled the pond to ensure impermeability. The next morning the water level had lowered. MSHA was notified immediately. No soil disturbance or material failure was discovered.

Struck by falling object

Placing a piece of steel in chute it fell striking him on the left calf

Struck by flying object

While removing sheave with a hammer a piece of metal chipped from hammer & struck right forearm. Metal imbedded arm causing one inch laceration.

On 3/8/the employee had been grinding on a crusher chute. He lifted his shield, removed his glasses, and wiped his face off with a rag and afterwards noticed something in his eye. He thought he got it out but when he woke up on 3/9/ his eye was red and irritated. He went to the eye doctor who then removed a piece of rusty metal from his eye and was given a prescription eye drop.

Non occ. non work related heart attack.On 2/3/12 a haul truck operator at the Quarry experienced chest and arm pain. EE's at the site called 911. The haul truck operator was transported to the local hospital via ambulance.

Fall to lower level, (Not Elsewhere Classified)

A body was found in the West pit area on 11/20/2012 at approximately 14:00 hours at the Flemingsburg Quarry. Investigation is ongoing.
