Bodily reaction, (Not Elsewhere Classified)

IE felt pain in lower back during pre-shift inspection when IE bent over. IE informed supervisor at 9:09am and was taken to hospital. Reportable due to restrictions, prescriptions and lost time. IE was transferred to different job based on restrictions and missed 10/27/17 & 10/30/17.

Caught in, under or between a moving and a stationary object

Injured EE and witnesses were changing a bend pulley on 50 conveyor. While performing the repair, the mast on the telehandler appeared to be lodged against the conveyor frame. When injured EE reached inside of conveyor frame to hook chain on pulley shaft, the mast moved, pinching left arm between conveyor frame and telehandler mast causing injury.

Struck against a moving object

Employee was injured while the haul truck was being loaded. Their left shoulder/lower neck injured when they were jostled around in the truck cab while rocks were being loaded in the bed. Type of injury is yet to be determined, but appears to be a type of strain.

The S-2 screen had stopped while crushing. The injured employee was removing the screen doors using a 16 inch spud bar. As the employee unhinged a door, the door sprung outward and struck the employee in left forearm.

Struck against a moving object

Emp was operating haul truck hauling shot rock to the primary crusher. While being loaded, the bucket of the loader contacted the truck bed and jerked the truck. The emp complained of back pain and told the super about incident but refused med attn at that time. On 9/21 the emp told the super was in a great deal of pain and needed to seek med attn for back pain.

Torque wrench was being used in a tight space making it difficult to remove from bolt.

Fall from scaffolds, walkways, platforms

At approximately 8:15 am (CST) customer driver climbed over a concrete barricade to retrieve their weigh ticket. When they stepped down, they fell on their left side. Person was treated and released with no restrictions.
