All incidents in 2023
While tightening a bolt on the crusher wear plates, the employees gloved hand slipped off the ratchet EE was using. EE fell back onto the other plates, fracturing EE's left femur.
EE was inside the primary crusher cleaning out large rocks. EE tweaked back while moving around. EE sustained a strain to the back and pelvis. Medical treatment was sought after.
A 4-person shot crew was loading a toe shot on a 147-foot highwall when a large rock in the wall failed and fell approximately 25 feet above the floor. The two people operating the loading hose ran, but rock rolled in the same direction as the IP. As the IP ran, IP tripped over the hose, brushed the rock, and fell. IP sprained right shoulder, elbow and damaged three teeth.
While walking up an inclined catwalk, carrying a piece of steel, the employee felt a sharp pain in left calf. The employee went to Urgent care, then to Orthopedic doctor. Determined EE had a tear in the calf muscle.
Walking on bench, foot shifted underneath EE from a loose rock, strain to right knee (Became Reportable on 2/26/24)
While taking a lunch break the employee was sitting on their lunchbox. when they stood up they lost balance falling backwards onto the floor. The following day they complained of knee pain and went to a clinic to get looked at. Resulting in restricted duty.
Lifting a screen panel with right arm outstretched below waist. As EE was lifting the panel, EE heard a pop in the right arm below the bicep and above the elbow. Employee used first aid processes until the arm started bruising. On 10/9/2023 EE was scheduled for an MRI. The MRI showed a torn Bicep on the right arm. Employee was then scheduled for surgery on 10/13/2023.
A loader operator was exiting the piece of equipment, a Kawasaki Z7 Front End Loader. When descending the ladder pan, EE missed the bottom step of the loader losing grip and falling to the ground. EE had used their right hand to break the fall.
Using a pry bar to free up a stuck stone. Employee's hand slipped contacting a steel guard. Contact resulted in a cut requiring 2 sutures.
While cleaning the tube over Portsmouth Rd. the tube collapsed and fell to the road. One the employees standing on the catwalk received a laceration to their left shin that required 3 sutures.
While using a hammer to hit a punch, the employee missed striking left thumb. This resulted in 9 stitches and a fractured bone at the end of the thumb.
An employee had an impactor liner, approximately 70 lbs, setting stationary on the impactor platform while changing worn curtain liners in the machine. The liner became imbalanced and began to fall to the catwalk. The employee moved to reposition the liner but was unsuccessful causing a sprained lumbar in EE's lower right back area.
