Accident type, without injuries

Weathering due to freezing & thawing. Had to move life line over into #3 entry, while rock was cleaned up and top supported. After clean up life line was re-run back to #4 belt entry.

Victim was inby last row of support when a rock fell on him causing death.

Injured jumped down approximately 3' to ground twisting his right knee.

The employee was working on the miner, he was lifting a lid up and pulled a muscle in his lower back.

Employee was installing a roof bolt and bent it. While trying to straighten it, he hit the rotation lever and spun the bolt twisting left wrist.

Employee was pulling pin out of H-Link on Hitachi 1900 Excavator when H-Link moved to right causing it to fall striking right foot. Causing severe bruising and swelling of foot.

EE was roof bolting while drilling a hole his wrench bent allowing the drill steel to come out and hit the EE on the right arm. Above the wrist causing a knot to come up on it.

Over-exertion in lifting objects

EE was stocking shelves in the warehouse and was lifting a box of parts when he felt a muscle pull in his stomach. Later in the shift a knot appeared on stomach.

He was helping co-worker load garbage in scoop bucket, while pulling on bent roof bolts one was hung and when it came loose struck him on the right side of the mouth chipping 2 teeth.

Struck against stationary object

Miner was walking around roof bolter when his arm touched the canopy of machine. A burr on the canopy cut his right arm.
