A non-injury unintentional roof fall occurred at Crosscut 59 in the #4 entry face area of the 002MMU. The fall is approximately 18 feet wide and 18 feet long and 8 feet high. There were no injuries associated with this roof fall.
On 8/28/2019 a Non-Injury Roof Fall occurred at this mine. The roof fall is located in the No. 6 entry at x-cut No. 24 on the 1-South Mains. The MSHA Hotline was notified and MSHA has conducted an investigation of the Roof Fall. The Roof Fall is approximately 18' feet (There are no injuries associated with this Roof Fall).
A non-injury roof fall was found in the No. 6 intersection and the No. 6 right crosscut at crosscut No. 7 on the MMU No. 002-0 section on the 1-South Mains. The roof fall is approximately 20 feet wide x 35 feet long x 12-16 feet high. The roof fall will be cleaned up and the mine roof re-supported. The MSHA Hot-line was notified of the roof fall. Employee was hanging roof bolter cable when a piece of rock fell striking them in the mouth, cutting upper lip on the inside.
Employee was roof bolting, while putting resin glue cartridge in the bolt hole. it burst and some of the glue got in their right eye.
The injured employee was on a small ladder working on the highwall miner hose reel. The base of the ladder kicked out and the employee fell injuring right shoulder.
Employee was roof bolting. While starting a new hole, EE stuck head out from the canopy to see if the steel was straight when a piece of rock fell and hit EE in the face cutting EE's upper lip which required 4 stitches.
Employee was loading roof support material on the roof bolter. EE bent over to pick up roof bolts and a small piece of rock fell from the roof and struck EE in the back of head causing a small cut to the left side of the head.
