Accident type, without injuries

A Roof Fall was found during a weekly examination. The Roof Fall is located on the 1 West Panel off the 1 South Mains. The roof fall is approximately 50' ft. long x 18.5' ft. wide x 6' ft. thick. The roof fall did not affect the mine ventilation or impede passage. The roof fall is located in the outby area of the mine. There are no injuries associated with this roof fall.

Caught in, under or between a moving and a stationary object

The employee was working on a belt head drive removing structure off the head drive when one of the rollers pinched the left index finger which resulted in a small laceration and a open fracture to the left index finger.

Over-exertion (Not Elsewhere Classified)

Employee was stepping out of the man basket to get extension cord twisting his knee. ""Left Knee""

Contact with hot objects or substances

Employee was changing a torque shaft on the gathering head of the 14-15 miner. When he pulled the broke shaft out it had built up pressure and it sprayed hot oil out striking him on the neck and face.

Over-exertion (Not Elsewhere Classified)

Employee was shoveling loose coal and rock up, when he felt a sharp pain in his left knee and couldn't put weight on it.
