FOREIGN BODY IN EMPLOYEE'S EYE. BEING REPORTED AS REPORTABLE WITH UNKNOWN PRESCRIPTION CHARGE RECEIVED IN MAIL YESTERDAY.

Foreign object in left eye while cleaning scale off center of pier. Wind blew object into eye.

Struck against stationary object

Was pulling wire when pull wire broke. On the follow through right hand hit the edge of the pull box. Cut right hand on top of index finger.

Cut left index finger. Whild holding a liner bolt head with left hand and using an air gun with my right, the bold head spun cutting my finger.

7000-1 FILED AS PRECAUTION. EMPLOYEE HAS A RASH THAT THE SOURCE OF HAS NOT BEEN IDENTIFIED. CONSULTED WITH LOCAL MSHA OFFICE AND WAS ADVISED TO FILE 7000-1.
