Injured EE stepped on side of rock causing left knee to twist resulting in a sprain.

Lifted a 5-gallon bucket of clinker. Note: Employee's injury occurred on 10/15/2007 as reported, but the employer was not informed of the injury as a reportable issue until 10/30/07.

Employee was exiting a loader, and lost his balance on the last step. As he fell, he caught himself sharply with his right hand on the handrail, and his right arm and shoulder took the shock of his fall.

Struck by flying object

EE received a foreign object in his right eye when he was walking through the plant, and was caught in a dust devil/windstorm. He was wearing safety glasses, but the wind blew a fragment foreign object into his right eye.

EE was using a knife to open a cardboard box containing filters. His hand slipped, while cutting the box, and he cut across his left index finger, between the 2nd and 3rd knuckles.

Doing demo to a chute - cut a piece loose and it flipped the wrong way and hit him in the hard hat.

Caught in, under or between a moving and a stationary object

Employee was using a rivetbuster, when the tooth got caught in the material. As he tried to remove it, the rivetbuster lunged forward catching his right ring finger and pinky finger between the rivet buster and material causing a 1 cm laceration to his 5th (pinky) finger.

Wind blown door to office closed on employee's left ring finger tip.
