Struck by falling object

Employee was repairing a shock/strut for T595 Bobcat Track Loader. EE lifted the cab, and while EE was working on the cab, it fell, hit EE in the head and then fell on EE's hand. Employees hand was crushed, and EE was taken to the emergency room where EE needed surgery.

Fall to the walkway or working surface

While walking across the yard at 04:05AM, employee tripped over the forks of a forklift and fell to the ground. Due to low lighting in the area, employee was unable to see the forks sticking up 1-2"" from the ground. Employee worked the rest of shift without concern but was later seen at an occupational clinic and was given modified work restrictions.
