Accident type, without injuries

A fire started at the Screening Tower at Quarry. First responders were called immediately. The fire burned for approx. 90-120 minutes. Investigation and repairs are ongoing. Damage is currently unknown.

Accident type, without injuries

The fire was in our cooler system. Equipment failure caused part of the flow of material to plug. With hot material spilling there was a small fire at the bearing. It was behind guarding. The fire caused a lot of smoke and the fire department showed up. They were not needed.

Caught in, under or between a moving and a stationary object

Guiding the rope during stripping covers off for the vulcanized splice, resulting in amputation of the pinky finger tip.

Fall to the walkway or working surface

EE slipped on the walkway at the bottom of the stairs due to an intense rain that had just occurred at the plant. EE did not request medical treatment at the time of the incident. The EE was under first aid care from the time of the incident until physical therapy was prescribed due to lingering shoulder pain. The first physical therapy session was March 6, 2019

EE was using a self-retracting knife to cut zip-ties. The knife slipped and cut through the employees glove and sliced EE's right index finger requiring 6 stitches.

Diabetic shock, was feeling bad, ate some candy then went into convulsions.
