A roof fall was discovered at the beginning of the shift on the main line belt conveyor 11 crosscuts inby the portal. The fall measured approx. 20' long by 8' wide and 6' thick at its peak. The fall was removed and the area was supported with cribs and cross collars.

A ROOF FALL WAS DISCOVERED AT APPROX 6:20, THE BEGINNING OF THE SHIFT. THE FALL WAS LOCATED INBY THE PORTAL BETWEEN THE #2 AND #3 CROSSCUT. IT IS DETERMINED THE FALL OCCURRED SOMETIME AFTER 3:30PM THE PRECEDING DAY AND 6:00AM THE FOLLOWING DAY. THE AREA MEASURED APPROX 3 1/2' X 12' X 12' AND WAS CLEANED UP AND CRIBBED
Injured stated that on 12-10-05 he was kneeling down to shovel some material when as he knelt down he struck his knee on a rock. He had no lost time until 12-14-05.
Injured stated he was shoveling loose coal from under the conveyor belt and when he mised a shovel full to empty on the conveyor belt, he felt a sharp pain in his lower back
Injured stated he was drilling a hole in the mine roof when he hit a crack and let his drill steel come out of the chuck stirking him on the fingers.
Injured stated he was walking near the scalehouse and fell landing on his knee. He had no lost time until 10-5-05. He left work and never returned.
Injured was bolting top when a rib rolled striking him on the right ankle fracturing his ankle.
INJURED STATED THAT AS HE WAS MUCKING COAL AROUND THE HEAD DRIVE HE BUMPED HIS ELBOW ON THE HEAD DRIVE AND WAS SHOCKED. HE WAS TAKEN TO THE DR AND CHECKED OUT AND FOUND TO BE ALL RIGHT. THE HEAD DRIVE WAS EXAMINED BY THE COMPANY ELECTRICIAN AND AN ELECTRICAL INSPECTOR WITH THE STATE MINE SAFETY AND LICENSING AND FOUND TO BE IN SAFE WORKING CONDITION
Injured stated he was traveling down along side the conveyor belt when he stumbled and fell striking his wrist on j-bolt which holds the belt structure together. The bolt cut his wrist, requiring 4-stitches.
