Accident type, without injuries

A rock fall was discovered on 11/5/07. The fall was located in roadway going to bleeder. The rock fall was approx 90' long in all 3 entries. The fall was from 3' to 8' high.

A rock fall was discovered near end of shift on 9-27-07. The fall was located 7 crosscuts inby the portal. The fall was approx. 30' long x 20' wide by 0'-3' high. The fall was located in the return aircourse.

An unplanned connection was made to old workings of an adjacent mine. The workings produced a minimal amount of water with no hazardous gases.

A roof fall was discovered approx 200' inby the portal in the return aircourse. The fall occurred sometime during the off shift between 7-31-07 and 8-1-07. The fall measured approx 12' wide by 40' long (extending into the crosscut) and 4' in height. The area was cleaned up and dresupported with cribs, collars and roof bolts.

During the examination of the no.1 conveyor belt it was discovered that the mine roof had set down & broke some cross collars that had been set along with cribs to support the area making the area unsafe for travel. The area was located at the 3rd crosscut inby the portal & measured approx 8'L x 8'W with the thickness undetermined due to the fall being caught by the cribs.

A rockfall was discovered during a weekly exam in an isolated area 2400' underground in the #4 entry of five entries. The fall had no affect on ventilation and/or travel and measured approx. 70' long by 20' wide by up to 5' thick. All entrances to the area were cribbed and dangered off.

Struck by falling object

Employee was helping the electrician put a lid on the power center. The lid dropped on employee's left hand index finger. It injured the area from the last knuckle up. The doctor later removed the tip of his finger from the last knuckle up.

The employee was taking structure out of the belt when a rock fell out from between the bolts and allegdly struck him on his left hand.

The employee was shoveling the #3 head drive when his shovel became hung in the belt splice and he allegedly twisted his back.

Caught in, under or between a moving and a stationary object

The employee claimed he was getting out of the scoop when a rib rolled off and allegedly pinned his right leg against the frame on the scoop.

Over-exertion in lifting objects

The employee claimed he was picking up crib blocks on the outside of the mine and allegedly strained his upper back.

The employee claimed he was getting a rock out of one of the bridges and allegedly strained his lower back.

Lifting to much weight in low coal Alleged strained upper back.

Struck by falling object

EE was removing his drill steel from the hole he had just drilled, when the last piece of steel fell from the hole, stiking EE on the middle finger on his left hand, fractured the finger. He has three lost work days.
