Struck against a moving object

Injured was in the bridge conveyor with a come-along tied to the bridge pulling it over to reload it onto the carrier when the bridge was accidentally started causing injured to fall injuring his shoulder.

Struck by falling object

Injured stated he was sitting on the mine floor along the side of the deck of the mobile bridge conveyor he operates when a slab of drawrock fell from between the roof bolts striking him on the head. He was taken to the doctor where he received stitches to his ear and was released.

Struck by falling object

Injured was constructing a brattice when a block that he had stacked fell and struck him on the left index finger, fracturing it near the tip. He left the job at 3:00 pm that day and has not returned.

Struck by falling object

The EE was bolting, when a piece of rock fell from the top and struck EE in the lower right arm. The rock itself was about 12""wide and 12""long, resulting in nearly a 2"" long gash in the EE's arm. EE was taken to the Dr. to have stitches.
