In the No.4 Entry (primary escapeway) between x-cuts No.4 and No.5 going underground from the surface, there was a rock fall measuring 15' wide, 8' long, and 4 1/2' thick.
#3 Entry (belt-line)- Around 2:20 P.M. on 3-31-11, A large piece of rock measuring approx. 16' wide, 10' high, and 2' thick, fell off the highwall knocking down part of the canopy and the Main Belt line.
During the weekly exam of air counts a rock fall was discovered on 3/9/11 at 6:25am in the #4 intake air course. Approx 20' inby the mine fan. The rock fall is approx 10' wide X 8' long X 4' high.
Employee was watching the cable on the dolly when the rib rolled off crushing him. Rib measured approximately 82 by 36 by 11 inches.
The crew was moving power up. EE was pulling Crawler cable slack, when he pulled something in his back.
He was standing on a ladder, when a shuttle car cable knocked him off the ladder, landing on left leg.

Employee was cutting a band that was around a bundle of timbers, when the band broke the timbers rolled and pinned his leg against the scoop bucket.
Employee was backing out of a shed on a three wheel buggy and pinched his hand between the shed door jam and the buggy steering handle cutting his hand and requiring stitches in his left hand.
Employee was pushing buggy up a hill, the buggy slipped and lost traction pinning the employee between the buggy and the belt structure.
Employee was operating walk thru roof bolter, as he was stepping from the rear of the machine onto the mine floor he twisted his left knee.
Employee was operating a Roof Bolter, while installing outside bolt a piece of rock fell striking him on the left hand.
EE was dragging a 18 H.P. Pump that weighs approx. 300lbs. off # 6 panel approx. 800 feet. EE hurt the lower part of his stomach.
He was shoveling on #2 beltline, as he leaned forward, his face was too close to the moving belt. The edge of the belt at a splice came in contact with his right eye. He had dirt in his eye and a scratch on the side of his eye.
Victim was a security guard on duty at RB #5 mine and shot himself in the head with a handgun.
