A brow fell and it was identified by employees during the shift. The fall damaged one side of a conveyor and discharge line for the pumps. No-one was injured and no-one was around when fall occurred.

When the BTI powdertruck was parked at the end of the shift, a small spark was seen by 2 employees under the operator's seat. All employees evacuated the mine and the equipment operators reacted by extinguishing the flame with 2 fire extinguishers. MSHA was called and communicated to us to stay out of mine.
Original incident was reported to company on 4/16/2010 but not MSHA reportable/lost-time until 9/14/2010, thus the submission of the report. Incident occurred when climbing down from ladder when his foot/hand slipped off of handrail. Employee caught himself on safety chain on separate piece of equipment parked next to loader.
Underground facility, shop area. Nothing within the conditions contributed to this accident.
While entering office trailer, employee slipped, tripped and fell on steps leading into office trailer.
While wearing the proper PPE required for the job, the employee developed a rash on his leg due to the friction of his boots on his skin (leg).
