Over-exertion in lifting objects

Lifting 50lbs. anfo bags from truck and loading them into shot holes over a 2 day time frame, approx. 30 bags each day. Causing back sprain to lower back.

Caught in, under or between a moving and a stationary object

Employees were maneuvering a full 55 gallon drums into a containment area. When placing a drum, the employee's finger was in the line of fire between the drum being moved and one already in the containment. Consequently, two fingers were impacted the middle finger losing the tip and requiring minor surgery and the ring finger which was broken both on the right hand.

Over-exertion (Not Elsewhere Classified)

While stepping out of transmixer truck, stepped down with right leg on loose rock in roadway causing employee to slip down ramp and when boot caught traction and stopped abruptly this caused a jamming motion to right knee.

Struck by... (Not Elsewhere Classified)

Detached a plugged 3' long 3"" to 2"" reducer from our concrete pump. Stood vertical to unplug, when tapping reducer on the ground to remove concrete, the concrete shot up the pipe striking the employee in the face, forcing the employee's goggles up and covering the employee's right eye with concrete.
