Employees arrived for the shift to find a backhoe on fire. Cause of fire was determined to be a lightning strike.

Fall from scaffolds, walkways, platforms

Employee was injured when EE fell through an elevated platform in a restricted area of the underground mine after EE climbed over a berm to access the restricted, inactive area in violation of the mine's rules and EE's training. The employee was not authorized to enter the restricted, inactive area and was not performing any work activity at the time the accident occurred.
