The #5&6 Kiln Burner was on the burning floor cleaning when EE heard a what EE presumed unplanned ignition in the #5 coal mill. The #5 coal mill was running normally prior to the ignition and continued to run after the ignition. The mill was inspected. No major damage was found. The mill was started again and continued normal operation. The cause of the ignition is unknown.

At approximately 11:30pm an employee reported seeing flames at the #1 Kiln Auxiliary Motor. The fire department was called and the fire was extinguished by 12:15am. No Injuries or Illnesses. The motor was damaged and sent out for repair. The cause of the fire is unknown at this time.
Emp a contract worker repairing a Scaler in Mod 12. The emp had fallen to the ground in fetal position, locked arms, clenched fist and bleeding from the ears. The witness did not know if the emp fell from a height or ground level. The injured emp was airlifted. He was lacking oxygen to brain for 10 minutes. He was pronounced dead at 9:45AM on Feb 28.
Employee was changing jaw dies & had his hand between a crescent wrench & a piece of angle iron. Employee asked another employee to kick the end of the wrench to break the nut free. When the employee kicked it the nut spun quick & the employee's left ring finger was smashed between the angle iron & the crescent wrench. The tip of the employee's left ring finger was amputated.
EE was replacing impeller bars in Crusher. While exiting crusher, the wedge locking the rotor in place came loose. The rotor appears to have been out of balance due to a missing set of impeller bars. The unbalanced rotor begun to spin & the EE's boot caught between the wall & the rotor pulling him downward. EE suffered left leg lacerations & two crushed toes on right foot.
While getting off John Deere loader that had been parked at the shop in a well lilt area, he began to dismount the loader and put one foot on the top rung and went to step down with the other foot, and lost his grip on the hand rail with his left hand. He swung around, but couldn't hold himself. He fractured his right pelvis. Should return to work in 4-6 weeks, per his doctor.
Employee alleges he was rolling a piece of screen cloth out on a storage rack and felt a pull in his back. He was diagnosed with a muscle strain, but after no improvement was sent for an MRI which revealed a herniation of a disc.
Moving pallet box to rear of service truck, strain/tear in right arm bicep.
At approximately 7:20 a.m. on Tuesday, May 13, 2014, an electrician was in the process of pulling wire through a 3/4"" electrical conduit from a junction box to the belt scales. As he pulled, his left foot slipped and he started to fall but was able to catch himself and regain his footing, but he twisted as he did, and he felt a pop in his back.
Employee was climbing down from a ladder that was on un-level ground when he fell to the ground. He braced his fall with his left arm causing pain in his left shoulder. The orthopedic doctor finally did a MRI and let us know there was a rotor cuff tear the end of August.
While coming down a set of stairs the employee says she heard a pop in her knee and felt pain. Initially this claim was ruled as non-work related and the employee did not allege a hazard. Since then the claim was accepted by worker's compensation and we are now reporting the claim.
Employee lost his footing on a low boy trailer and slipped causing a strain to his lower back.
Employee exited the service truck to set the wheel chocks. The employee opened the door put his left leg out and then stepped out with the right leg. Employee slipped forward causing the employee to fall and injure his right calf.
While working at East Lime Loadout, a 2nd shift Vac Truck employee stumbled over his boots and fell onto a handrail. The right side of his chest and stomach made contact with the handrail causing him pain. The employee was evaluated at the ER and given restrictions.
The employee was cutting urethane chute liner when the utility knife slipped out of the cut. This resulted in a laceration below the right thumb. He received 9 stitches.
The employee was lifting a garbage bag full of dust bags out of a baghouse when he felt pain in his back. The employee requested medical treatment and was placed on restricted duty at this time.
Employee was using an extension ladder in order to patch a hole on the sand screw chute. Ladder shifted and employee fell about 6ft on his right foot and broke heel bone on his foot.
Miner was working in close proximity of a welding operation performing another task and over the course of the day contracted flash burn in both eyes.
Employee was using a prybar to assist in moving a gate handle. The prybar slipped causing a contusion and fracture to the left thumb.
Employee strained neck by turning head to look behind him while backing up in his haul truck. PT, Prescriptions, and Restricted Duty occurred. At the time of the incident the mine operator did not believe the incident to be work related. We are now reporting as the incident has been accepted by Worker's Compensation.
While assembling loader tire bead band became bound. Trying to seat the band the tech struck the top portion of bead band and lower portion sprung back striking tech in the upper abdominal area.
Employee was attempting to slide an aluminum pan underneath a rail car to vacuum out material. While working a crouched position the employee attempted to move the pan by twisting at the torso and felt a pain in lower back. The employee was placed on restricted duty to allow for the lumbar sprain to heal.
Brick layers were re-bricking inside #2 rotary kiln while coworker was adjusting mechanical tensioner supporting the brick, when a single brick slipped and fell approximately 3.5 feet onto the injured employee's left hand causing a fracture to the finger. Return to work with restrictions the following scheduled shift.
Employee called to report that on Friday, 2/14/14, @ 1000 am she was getting a sample for a customer using a 5 gallon bucket & when she went to pick up the bucket to place on tailgate of truck she felt a ""sharp pain in the front of the right shoulder that shot shooting pains down the arm to the fingers & up the neck"".
Employee was using a spud bar to break up frozen material in the dust bin under lock out conditions. The employee reported shoulder pain shortly after, but continued to work as scheduled. EE continued to work his full duty until the shoulder was surgically repaired, after which he reported for restricted duty on his next scheduled shift.
Vac Trk Opr. was cleaning spillage in N/S Lime Tunnel when overhead 4"" vac hose came off the attachment. Loaded hose knocked off safety goggles and caused lime to impact his left eye requiring medical attention and several restricted days to heal.
EE reported that while running the dozer he backed over a frozen dirt clod which caused him to jar his back. Health Bridge was contacted. EE was informed by Health Bridge that home care was all that was needed. Ice and Ibuprofen was recommended. January 5th EE saw a doctor and was given a restriction of not operating the Dozer for 2 weeks.
Technician was mounting an oil cooler assembly onto a CAT D400 truck. Had to use a pry bar to help get the cooler mounting bolts lined up with the bolt holes. The pry bar slipped off of the cooler, forcing the tech's left hand to hit the head of another bolt on the machine, lacerating the top of the knuckle on his middle finger. Also, the rain made all of the equipment wet.
While working on the crusher feeder employee slipped, pinched his finger between cap he was holding and chute work. Employee failed to report the injury until the next day after seeing his personal doctor, who treated it as a first aid. On 9/17/04 we received notification of a fracture to the tip of his finger, which made this a reportable injury.
Employee was cleaning the magnet off at the secondary plant. As he was trying to remove a piece of metal, the magnet pulled it back and pinched his thumb between the magnet and the metal. He was wearing his gloves and all other PPE required for the task. *he received five stitches making this a reportable medical.
Worker was changing the bit on the drill. He cut his thumb on a barb on the surface of the bit.
EMPLOYEE WAS HOLDING ROPE LOOPED IN HIS HAND TO CUT WITH UTILITY KNIFE. WHEN EMPLOYEE TRIED TO CUT THE ROPE THE KNIFE SLIPPED OFF THE Rope, came TOWARDS THE EMPLOYEE, AND LACERATED HIS LEFT ARM. THE LACERATION REQUIRED 7 STITCHES.
Employee was helping lift a 2 1/2"" inch woven wire screen on the top screen deck . When had lifted the screen approximately 12"" to 18"" off the floor, he felt something pop in his left arm. He was taken to the doctor that day, who diagnosed it as a strain. He was returned to regular duty.
TECHNICIAN WAS LIFTING AN ENGINE FLYWHEEL OUT OF A FLYWHEEL HOUSING. OVERCOMPENSATED FOR THE WEIGHT OF THE FLYWHEEL AND LACERATED THE TOP OF HIS RIGHT HAND ON THE FLYWHEEL HOUSING, REQUIRING 3 STITCHES.
Wind gust stirred up dust that got in the employee's eye while walking. Notification that employee had gotten medical treatment, outside working hours, (eye flushing, antibiotics) wasn't received until 9/25/14.
Employee was cutting a section of rubber water hose with a utility knife and cut his arm requiring sutures.
Employee was ducking under a conveyor to walk from one side to the other. He came up short of the clearance and hit his right ear on a crossbar causing a laceration to his ear requiring stitches.
While putting material in QC pans, EE was picking them up when the material shifted causing the pans to tilt and slide across my fingers cutting three fingers requiring 5 stitches.
Employee was checking stockpile with left foot on the ground. When she twisted to move to another location her knee gave out. Knee immobilizer was issued and she is required to wear.
Pole was picked up on site and on the pole trailer and transported. In the course of lifting pole off trailer the strap failed and pole kicked up and struck our ee on left side of his mouth and face. EE had cuts below his lip and to his tongue and had a slight fracture to left cheek. Oral surgeon said fracture would heal on its own and the ee had stitches below his lip and tongue.
Received results indicating employee reached the MSHA levels of silicosis.
After the shift at New Harvey EE was alone in the scale house. The leadman had already left for the day. EE had chest pain strong enough that he called his family to come and get him. EE was taken to the hospital, they confirmed he had multiple heart attacks.
Employee reported severe pains in upper stomach / lower chest area. He had been operating a skid steer prior to feeling bad. He was taken to the hospital by ambulance. It was determined that he had a bad reaction to medication, Predazone, a medicine his family doctor had given him for poison ivy/oak. Employee felt he got the poison ivy/oak from his dog.
Trespassers were operating an ATV near or on mine property. The ATV overturned and as a result we were notified that one of the trespassers was killed.
