Accident type, without injuries

While scaling loose rock with the scaler, rock came down on the transfer of #2 & 3 belts. The area was approx. 8 ft. thick x 25 ft. in diameter.

Fall from machine

Employee jumped approx. 4 ft. from man backet to ground - felt pain in left knee.

Struck by powered moving object

Employee was manually operating elevation measure stick. Trackhoe bucket hit him - pushed him into a dirt wall. He ended up with dislocated shoulder and sprained ankle.

production support operator reported that he had twisted his right ankle on Sunday, May 23rd getting out of his loader to refuel at the North fuel station. He stepped down onto a rock causing his foot to twist. X-ray of the right foot/ ankle revealed a new fracture.

Fall from machine

Ee went to get product from the truck. the bench was muddy and he had mud on his shoes. he stepped on the bumper of the truck with his left foot and it slipped off the bumper. he hit his knee and fell backwards landing on his back

EE WAS HOOKING UP BUZZER AT PRIMARY CRUSHER. HE SAW THAT A HAUL TRUCK HAD ALMOST RUN OVER A SHARP, POINTED ROCK. EE WENT TO MOVE THE ROCK. WHEN HE CARRIED THE ROCK OVER AND PITCHED IT OVER THE BERM, HE FELT PAIN IN HIS LOWER BACK.

IN AN EFFORT TO CLEAN THE RIGHT CAB MOUNTED MIRROR OF A CATERPILLAR 775B TRUCK, THE OPERATOR EXITED THE CAB AND ATTEMPTED TO CROSS THE HOOD IN ORDER TO GAIN ACCESS. WHILE CROSSING HE SLIPPED AND FELL TO THE GROUND CAUSING THE INJURY.

Struck by flying object

Employee was washing tail pulley of belt. He was positioned below the jaw crusher below and protective deck. A rock virbrated through an opening below the protective deck, falling and hitting the employee.

WHET TO REPAIR VSI. OPENED FIRST DOOR AND AS HE WAS OPENING THE SECOND DOOR, THE FIRST DOOR FELL AND HIS HAND WAS SMASHED BETWEEN BOTH DOORS.

Quarry loader ee was operating the 992 g loader on 5-26-04 and reported running into large toe at the highwall face of approx 1030 am. he stated the loader stopped hard pitching him forward against his lap belt causing soreness in his lower back. he continued working as scheduled until his back pain became worse. on 6-25-04. additional medical treatment. restrict days off

EE WAS LOADING A PRODUCTION SHOT. A 5 1/2"" STICK OF BLASTING AGENT GOT HUNG IN THE HOLD APPROXIMATELY 2 FEET FROM THE TOP. HE LAID DOWN ON THE GROUND AND REACHED DOWN INTO THE HOLE TO RETRIEVE THE STICK. HE HURT A MUSCLE IN HIS SHOULDER/NECK AREA.

WHILE HOLDING A 2"" X 4' BOLT WITH HIS LEFT HAND, EE WAS TRYING TO UNSCREW THE NUT WITH HIS RIGHT HAND. THE BOLT SLIPPED AND SMASHED HIS MIDDLE FINGER.

Struck by falling object

Two employees were carrying an oxygen tank with injured employee walking backwards while looking over his shoulder. He stumbled backward and fell on his rear. The oxygen tank fell between his legs onto his finger and smashed it against a rock, which split his finger open. The employee was wearing gloves.

Employee picking up a metal stake, when he dropped it. He tried to catch it and smashed his finger between the stake and edge of concrete.

Struck by falling object

ERECTING SCAFFOLDING TO SUPPORT DECKING WHEN HE WALKED PAST THE I-BEAM SUPPORTED ON THE SCAFFOLDING (5'HIGH), HE BUMPED THE I-BEAM & IT FELL & STRUCK HIM IN THE LOWER BACK OF HIS HEAD.

BRACING OFF AN 18' WALL WITH 24' LONG 2 X 4'S AS THEY WERE LIFTING THE 2 X 4'S TO LEAN AGAINST THE WALL FOR ATTACHMENT, THE 2 X 4'S ROLLED AND EMPLOYEE STRAINED HIS WRIST.

EE was driving anfo truck with door open. When he went around a turn the door closed fracturing his left hand.

Driver of the troop carrier went too close to the edge of the road and tipped the vehicle containing employees over on driver's side into ditch.

EE WAS RUNNING A LOADER. THE AC VENT COVER CAME OFF. HE REACHED IN TO RETRIEVE IT AND HIS FINGERS WERE CUT BY THE FAN.

EMPLOYEE PARKED SERVICE TRUCK ON A SLOPE. WHEN HE OPENED TOOL BOX AND LET GO, TOOLBOX LID FELL CATCHING EMPLOYEE'S FINGER, ONE NEXT TO LITTLE FINGER ON LEFT HAND. IT CUT FINGER ON TIP PART. I CARRIED EMPLOYEE TO HOSPITAL. THE CUT REQUIRED 5 SUTURES. EMPLOYEE RETURNED TO WORK WITHOUT ANY OTHER MEDICATION REQUIRED.

EE WAS SITTING IN EMPLOYEE BUILDING. HIS MINE BELT WAS ZIP-TIED TO A CHAIR BY ANOTHER EE IN A ACT OF HORSEPLAY. EE CUT HIS HAND WITH A KNIFEWHILE TRYING TO CUT THE ZIP TIE.
