Accident type, without injuries

Upon arrival to mine site, employee noticed a broken electrical coop power pole was on fire. Coop deenergized line and fire went out.

A break in a slurry line caused an inundation of water in the mine. This condition was discovered on a regular workplace exam before the beginning of the shift. Only two miners were in the mine performing the workplace exam and they safely evacuated to the surface. There were no injuries.

Flames were noticed on the truck by the blasters when prepping the face to be loaded with explosives. Mine was evacuated.

A miner was involved in an accident while operating a CAT 777D haul truck that resulted in the truck being turned on it's side. No injuries were reported.

Based on investigation to date, employee suffered a personal medical event (cardiac arrest). ""Per Prelim: After washing a concrete pad, a miner drowned in a two-foot-deep pool of water.""

The miner was performing maintenance on a crusher; in the process, the miner came into contact with a sharp edge. This resulted in a laceration and partial amputation on the tip of the left ring finger.

Employee reported pain with shoulder after adjusting a gate at the secondary plant. It was determined on 11/19/2024 by an Orthopedic Doctor that the employee needed physical therapy.

Employee was lifting a wooden block and injured their bicep. Injury date occurred 9/23/24 but injury did not become reportable until 10/30/24.

Miner states was bit by a tick on the back of the neck while inspecting fire extinguishers at the pug mill located at the O'Neal Quarry on 06-13-2024. EE was absent from work on Monday 06-24-2024 and has not returned to work.

Operator was helping pull a cable that was caught in the tracks and while EE was pulling, EE turned and pulled a muscle in EE's back.

EE was washing out in the surge tunnel. While pulling the hose to reposition, it got caught around a pile of dirt and pulled EE's left arm. Injury date occurred on 10/16/2024 but injury did not become reportable until 1/13/2025.

Contractor was changing V-belts on #4 conveyor. EE was using index finger on their right hand and was walking the belt around the sheave. EE thinks the belt hung on the back stop, because it broke loose and EE's left wrist was run between the belts and sheave 2 times before EE could get it free.

While performing maintenance on a feeder, employee lifted an object and dislocated their shoulder.

Miner was using a heated water pressure washer to clean a transmission and while relocating the hose, EE shot hot water down into EE's boot. EE suffered a second degree burn to the ankle area and the skin peeled off.

On 9/12/2024 EE was walking down a set of stairs, slipped, and fell backwards, causing slight discomfort. EE received first aid care until 12/2/2024. On 12/2/2024 EE was prescribed physical therapy, making the incident recordable on 12/2/2024.

While performing cleanup duties, the employee turned body to take a step and dislocated left knee.

8/6/2024, EE experienced unspecified back pain while operating haul truck. 8/7/2024, EE was seen at clinic, first aid only. 8/22/2024, EE condition did not improve, occupational doctor prescribed physical therapy.

Employee was changing return rollers when the employee felt lower back pain. Physical Therapy and restrictions were prescribed by the clinic. Injury became reportable on 07/23/2024.

While performing maintenance on a crusher, employee stepped to swing a hammer and dislocated right knee. This injury became reportable on 7/30/2024 when physical therapy was prescribed.

Repairing crusher and got hand caught between spider and mainframe. This cause miner to have a crushing action on their right hand.

The mobile mechanic was preparing for welding activity by grinding and prepping work area. While using the grinder it kicked back and struck the mechanic on the right cheek causing a laceration.

Employee slipped on the concrete floor and fell on EE's left side. Physical therapy and restrictions were prescribed for the employee. Injury became reportable 4/26/2024 when the MRI validated the injury was work-related.

Porto power fell over and contacted right ring finger of employee.

Employee sustained finger fractures and lacerations while hanging geo fence underground.

Employee was walking to check on pit pump and tripped resulting in a shoulder strain. Accident became reportable on 11/15/2024 when employee had surgery.

Employee hit air line bracket when man-lift abruptly stopped.

12/10/24 EE slipped and fell on ice. EE continued first aid treatment until 1/27/2025 EE was prescribed psychical therapy.

Employee fell into the frame of a conveyor while removing tail pulley guard.

Operators machine was down. Mechanic was working on it. Operator rested right hand on stick while waiting. EE slid hand down and caught hand on a metal burr, cutting EE's palm.

Loading a fuel tank on a trailer. Ladder fell down injuring right index finger.

Wind blew material into the employee's right eye. Doctor removed material. Prescription medication was prescribed to the employee.

Surface drill operator assisting mechanic removing internals of a 4-inch down hole hammer (off the drill). Inertia movement of internal piston caused it to knock the internal sleeve out of casing, striking driller in forehead. Driller received 3 sutures and returned to work immediately.

Employee was reeling up a hose on a reel when employee felt a pop in elbow. After seeing an Orthopedic Specialist, physical therapy and restrictions were prescribed on 02/05/2025 which made it reportable.
