Mike was exiting his work truck, when his right leg came in contact with the ground he felt a twist and had problems putting weight on his leg.

Fall from machine

Plant @ #4 shaker. Employee was changing the top deck screen on the #4 shaker. Slipped with the screen and slid down the chute feet first.

Fall down stairs

EMPLOYEE WAS CLIMBING STAIRS CARRYING A PIECE OF METAL, WHEN HE DROP IT, HE WENT TO CATCH IT. HE SLIPPED AND FELL DOWN STAIRS. HE STRAINED HE RIGHT SHOULDER

Over-exertion in lifting objects

Helping another employee remove a 1/2 sheet of plywood from under a belt. When he picked it up to pull it out he heard a pop and felt a pain in his left lower leg. Pulled a muscle or possible tear to MCL or Achilles rupture.
