Caught in, under or between collapsing material or buildings

EE entered feeder when gate would not close. While cutting out a piece of metal that was blocking the slide gate, material sloughed off and engulfed the EE.

Fall from machine

Crew was unclogging the crusher when one of the large rocks came out of the rigging and struck the handrail, breaking it. The crew installed barricade tape and continued working. The IW was working on the platform and backed through the barricade tape and fell to the ground. Coworkers asked if EE was okay, EE responded that EE was fine and went back to work.

Over-exertion (Not Elsewhere Classified)

Injured EE and another EE were replacing a return roller on the carry back conveyor when the roller shifted. The injured EE moved quickly to hold the roller and felt a pain in side/back.

Struck by flying object

Tech was using a wire wheel, a bristle came loose and penetrated the skin in the thigh area on the left leg. Tech went to the hospital to have the piece removed.

Employee was cutting belt with knife and came in contact with hand causing injury.

Struck against stationary object

EE was tightening the plug on the gear box when the wrench slipped causing EE's left forearm to get caught on a piece of metal catwalk material.

Fall onto or against objects

Clamp failed holding counter weight to belt sled. Weight fell causing the belt to move while they were on it and employee was bounced back into the top belt.
