Struck against a moving object

Employee was driving an A40G haul truck with the seat belt on in a parked position when employee was rear-ended by a 45G haul truck traveling approximately 8MPH. Impact caused employee's body to shift forward and left shoulder hit the steering wheel resulting in a cervical strain.

Absorption of radiations, caustics, toxic and noxious substances

Injured employee (IE) was in the process of disconnecting the cam lock for a grout hose. As the IE was uncoupling the hose, the remaining grout in the hose, made contact in the facial area. After the IE removed their safety glasses, IE felt irritation in EE's Left eye. IE's eye was flushed onsite and IE was taken to the hospital for evaluation.

Struck by falling object

EE was carrying a 45 lb roller when EE lost control of the object causing it to fall, crushing EE's right index finger between the roller and the concrete. The finger was not broken but a prescription was given for swelling.

Struck by... (Not Elsewhere Classified)

EE was trimming palm trees when a palm frond sprung back and a thorn punctured EE's hand. First aid was performed but swelling occurred. Triage doctor referred EE to the clinic and antibiotics were prescribed. No further treatment was given.
