AN UNPLANNED ROOF FALL OCCURRED IN THE #4 ENTRY. 300' INBY SPAD #114 ON 5 BELT. APPROXIMATELY 20,000' FROM ACTIVE FACE. THE FALL MEASURED 70' LENGTH, 20' WIDTH AND 5' HEIGHT. NO INJURIES WERE REPORTED. VENTILATION WAS NOT AFFECTED.
An unplanned roof fall occurred in the #6 entry. The fall started at spad #16586 and extended 90' inby that point. The fall measured 90' length, 18' width and 6' to 7' high. No injuries were reported. All entrances have been cribbed off.
AN UNPLANNED ROOF FALL OCCURRED IN THE #5 ENTRY AT SPAD #1664 APPROX 4500 FT FROM ACTIVE FACE. THE FALL MEASURED 20'LX20'WX7'-8'H. NO INJURIES WERE REPORTED AND PASSAGE OF PERSONS WAS NOT AFFECTED DUE TO RE ROUTING OF SECONDARY ESCAPE.

An unplanned roof fall occurred int he #5 entry about 2 breaks inby spad #122998. The fall measured 30' x 20' x 7'. All entrances to the fall were breakered off and area will be rehabilited. There were no injuries reported but passage of person was affected.

AN UNPLANNED ROOF FALL OCCURRED AT THE LOGAN'S FORK MINE 15,000 FT FROM THE ACTIVE FACE OUTBY AT CROSSCUT 2.3 SPAD #243. THE FALL WAS 19'W AND 4-5 ' HIGH. NO INJURIES WERE REPORTED AND VENTILATION WAS NOT AFFECTED.

Injured was tramming toward stacker when the loader he was operating hit a hole and jarred the operator.
Member is on electricing, who was in the process of serving, pinning him to the ground. Rock measured 6'x3'x2' and fractured his l, thru l5 vertabral.
Member was started to drill on roof bolter when a piece of rock striking him on right legs. rock measure 31"" long 7"" wide 10"" thick.
EMPLOYEE WAS THROWING TRACK SWITCH WHEN HE TORE A BICEP TENDON.
Member was attempting to hang his buggy anchor, he was located on the left side of the feeder on the #2 section, as he was hooking the chain over teh anchor bolt with his other hand, he felt a sharp pain in his lower back. X-rays were taken and were negative but he was advised to stay off work until 10/17/05.
Unloading equipment and flag pole came out. Employee did not notice and jabbed flagpole in eye.
MEMBER WAS HELPING TAKE A LINK OUT OF A RIPPER CHAIN. HE HAD 3/4 TON COME-A-LONG HOOKED TO EACH END OF CHAIN. THE CHAIN SLIPPED OFF COME-A-LONG AND FELL AND HIT MEMBER IN THE UPPER AREA OF HIS RIGHT LEG. NO BROKEN BONES, BUT SOME SWELLING
Attempting to dismount meachanic's truck foot slipped causing him to twist right knee.
INJ BENT DOWN TO PICK UP A CRESCENT WRENCH, AS HE STOOD BACK UP HE TURNED TO THE LEFT. THIS CAUSED A P0PPING NOISE IN HIS LEFT KNEE. CONT'D TO WORK UNTIL 8/22/05. TAKEN OFF WORK BY DR FOR TEAR OF MINISCUS AND CARTILAGE IN LEFT KNEE.
Employee was leaving work in his personal vehicle. He was involved in a two vehicle incident involving a coal truck. Employee was diagnosed with whiplash.
Ee was machining faces on nose cone of a d10r dozer. I stopped the drill and steped around to look at face & I stepped on a small rock and twisted my knee as I stepped around to look at the face.
EMPLOYEE WAS LIFTING BAGS WHILE LOADING A BLAST. HIS FOOT SLIPPED CAUSING HIM TO TWIST. HE EXPERIENCED PAIN IN LOWER BACK

EE was carring a 72"" timber to load into a scoop bucket, when he stepped into a hole in the mine floor. this resulted in twisting his left hip.
EE was in the process of building a stopping when he stepped on a piece of cinder block. Twisting his ankle.
EE was assisting the continuous miner operator in hanging cable when he felt pain in his left shoulder. He continued to work until 4-4-05.
Employee was walking thru the 3 to 4 crosscut when he slipped in mud and twisted his left knee.
EE AND CO-WORKERS WERE INSTALLING A GATHERING HEAD IN THE HWM. AS THE POT DROPPED INTO POSITION, THE SLATE BAR USED TO GUIDE THE POT INTO PLACE BECAME CAUGHT. THE WEIGHT TRANSFER CAUSED THE BAR TO ""KICK BACK"" STRIKING EE ON THE RIGHT SIDE OF FACE
Employee bent down at the knees to pick up a 13"" length of 12"" pipe weighing about 100 pounds to place it on a table for grinding. As he stood up, he felt a burning sensation with the knee and tingling down into the leg which worsened over time. Employee did not seek medical treatment until 2/2/05 when the pain didn't subside.
EE DRIVING ROCK TRUCK UP OUT OF STEEP PIT WHEN THE LOAD SHIFTED TO THE REAR CAUSING THE TRUCKS FRONT END TO RAISE UP 4-5 FEET AND THEN SLAM DOWN JARRING THE TRUCK CAUSING PAIN IN LOWER BACK.
Member was carring his cap light in hand and tripped over his cap light cord falling off of tailpiece he was welding on, landing on his shouler.
Member was walking to section from end of track and stepped in a hole hurting his right knee.
Injured was traveling from the E2 to the E3 manbar, as he entered the E3 curve he took curve at too low of an angle, therefore trailer began to push the tractor across the curve. Thr driver tried to correct the situation, but failed. the truck overturned onto the driver's side.
Operating excavator, building temp. access road over to bench on refuse impoundment. Trammed onto rocks thought to be stable. When he picked up another bucket of material & started to swing around to place the material, the rocks under the tracks gave way & allowed the sides of the tracks to dig in. Excavator tipped over & rolled down the slope. Laceration to forehead.
EE with prior history of recurring back pain was climbing structural steel to make a connection on a 4-legged tower. While climbing, he felt a sudden burning sensation in his low back, but continued to work remainder of shift. His next shift on 4/4/05, he was still sore and went to doctor where he was diagnosed with low back strain.
MEMBER WAS PREPARING TO PUT HIS MANTRIP BACK ON TRACK THAT HAD JUST WRECKED. HE WAS WALKING FROM ONE END OF THE TRIP TO THE OTHER, WHEN HE TRIPPED AND FELL, PUTTING HIS RIGHT HAND DOWN TO CATCH HIMSELF, CAUSING A LACERATION FROM A TRACK TIE WHICH NEEDED STITCHES TO CLOSE.
Member was reaching to pick up a crib block, when another member who was helping him, threw a crib block in the same direction, which struck member in the left hand causing a laceration which needed 6 stitches to close.
Member was in the process of using a utility knife. to cut a piece of 48"" belt, when the knife slipped and caused a laceration to his left calf area, which needed 4 stitches to close.
Member was on the surface of an underground mines, in the process of uncoupling a supply can from on motor, when as the pin was moved the car shifted pinching his finger. 3 stitches were need to close the wound.
EMPLOYEE WAS USING 1/2 IMPACT WRENCH, TAKING OFF A NUT. WHEN THE NUT WAS FULLY UNTHREADED, THE AGITATOR TO THE DIFUSER FELL OFF STRIKING HIS RT. HAND CAUSING A FRACTURE.
MEMBER WAS FINISHING UP ON THE STOPPING HE WAS BUILDING WHEN THE STOPPING FELL OVER TOWARDS HIM AND STRUCK HIS RIGHT HAND CAUSING A LACERATION TO HIS PINKY FINGER, WHICH TOOK 3 STITCHES TO CLOSE.
Member was in the process of gathering materials from inside the supply shack on the surface. As he reached to pick up a shut-off valve for the waterline, a snake startled him and he turned to run. He fell and struck his face against a pallet of anchor bolts, causing a laceration.
MEMBER WAS LOADING ROCK OUT WHEN A PIECE OF ROCK FELL 12 FT HITTING HIM ON RIGHT HAND CAUSING LACERATION TO HIS SMALL FINGER THAT TOOK 9 STITCHES TO CLOSE
EE AND COWORKER WERE IN THE PROCESS OF MAKING A MECHANICAL SPLICE IN THE BELT AT #4A PONY HEAD ON #1 SECTION. EE WAS RUNNING HIS RIGHT HAND ACROSS NAIL BOARD TO MAKE SURE THE NAILS WERE BRAIDED, WHEN HIS COWORKER STRUCK HIM IN THE HAND WITH A 2 LB. FLEXCO HAMMER. EE WAS DIAGNOSED WITH HAIRLINE FRACTURE, RIGHT MIDDLE FINGER.
MEMBER WAS INSTALLING PIN TO COUPLE HWM CARS ON AN ADDINGTON HWM SYSTEM. AS HE INSTALLED THE PIN, HIF FINGER BECAME CAUGHT BETWEEN THE CAR FRAME THE LIP OF THE PIN. THIS RESULTED IN 2 SUTURES AND A FRACTURE TO HIS LEFT INDEX FINGER.
EE WAS IN THE PROCESS OF STRAIGHTENING TOP STRUCTURE USING A HAMMER TO ANGLE STRUCTURE. HE DERW THE HAMMER BACK, STRUCK HIS HAND AGAINST 4"" WATER LINE COUPLING AND CAUSED A FRACTURE TO HIS LITTLE FINGER. NO STITCHES WERE NEEDED, HE DID NOT MISS ANY DAYS DUE TO INJURY
Employee had come off of the loader he was operating to help remove rock from on top of the miner. He was pulling rock off of the side of the miner when his left hand was cut on a piece of slate rock. This resulted in 6 stitches.
Employee has worked in and around coal mines for 25 years. He has been diagnosed with an occupational illness (repeated trauma/degenerative disc disease) of the lumbar spine. He has not missed any scheduled work. This is a degree 6 occupational illness.
WHILE INSTALLING BOLTS EMPLOYEE LOST BALANCE AND STRUCK HEAD ON PUSH ARM OF D9 DOZER.
Employee was helping to change the drive line in the Tandem truck he was operating. As he was pushing the drive line into the yoke he sprained his right thumb.
MEMBER WAS HELPING CHANGE MINER BITS USING A HAMMER WHEN HE SWUNG AT BIT, MISSING AND STRIKING HIS THUMB ON HIS LEFT HAND CAUSING A LACERATION WHICH HAD TO BE STITCHED
Employee was on his knees on the ground over top of a piece of belt. He was cutting with his utility knife, when the knife slipped causing a laceration in his right leg, which needed stitches to close.
Employee was helping a co-worker make a splice on the 5C Overland belt. He reached to move speed loader and was struck on his right index finger by a Flexco hammer, breaking his right index finger.
EE WAS IN THE ACT OF PULLING A TANK OF WELDING GAS OUT OF O2/ACETYLENE CAGE, WHEN THE CAP CAME OFF AND STRUCK HIM IN THE FACE, RESULTING IN A LACERATION THAT NEEDED SIX STITCHES TO CLOSE
Employee had finished hooking up cat and tossed cathead into tray of addcar. Cathead bounced and contacted his left index finger, partially tearing off fingernail.
EE WAS USING A TORCH TO CUT CONNECTING LINK ON CONVEYOR CHAIN SO THE CHAIN COULD BE PULLED OF S/C. THE LINK FLOPPED UP EE USED RIGHT HAND TO FLPO IT BACK DOWN AND BECAUSE OF THE WEIGHT OF THE CHAIN, IT MOVED AND CAUGHT RIGHT HAND BETWEEN THE DECK AND CHAIN FLIGHT
EE WAS CLEANING UP A PIECE OF ROCK BESIDE BELT LINE WHEN A ROCK FELL (MEASURING 2' LONG X 3"" WIDE X 0-0.5"" THICK) LANDING ON EE'S LEFT HAND, RESULTING IN A LACERATION. 3 SUTURES REQUIRED.
WELDING ON AXLE SEAT & WHILE CLEANING METAL PIECE OF RUST GOT INTO HIS EYE.
EE WAS USING A UTILITY KNIFE TO CUT A PLASTIC ELBOW WHEN KNIFE SLIPPED AND CUT HAND/WRIST. 12 OUTSIDE AND 6 INSIDE STITCHES
Injured was walking across pit and stepped in a snow-covered frozen rut, resulting in a sprained ankle.
Member hand came in contact with another member's knife that was open causing a laceration, 5 sutures. Left hand small finger.
EMPLOYEE HAS BEEN DIAGNOSED WITH CARPAL TUNNEL IN BOTH WRISTS
Employee has been working in and around coal mines for 32 years and has been diagnosed with bi-lateral carpal tunnel syndrome. Surgery was performed on 4-25-05.
Due to occupational illness, as substantiated by medical records, employee became unable to work because of the ailments described in the medical records. This was not due to a single event at the mine, but repeated trauma over an extended period of time due to normal job duties.
Employee has been working in and around coal mines for 28 years. Has been diagnosed with carpal tunnel syndrome in the left hand. Surgery was performed on 4-21-05.
