Accident type, without injuries

An unintentional roof fall has occurred on the #3 section right return, 4 lt. Bear Sub mains #14 panel in by spad #(S) 10118, 10117, 10121. The fall was approximately 75ft X 20ft X 7.5ft. There were no injuries reported.

Over-exertion in lifting objects

Employee was lifting a 5 gallon jug of water and felt a sharp pain in lower back and right leg and thigh. Began missing work on 09/23/2016.

Injured was walking to the warehouse when EE tripped on a down drill that had been covered by the recent snow fall.
