Accident type, without injuries

An unplanned roof fall occurred in the #5 entry 1 break outby spad #4221. Ventilation was not affected, no persons or equipment were involved. Lifeline was rerouted. All approaches have been breakered off. The area will not be rehabilitated. Fall measured 22' long, 20' wide & 4' high.

An unplanned roof fall occurred at spad #2485. Fall was in the #4 and #5 entry. Ventilation was not altered, passage of persons was not altered. All entrances to fall were breakered off. Area will not be rehabilitated. Fall measured length 50', width 20', height 6'.

An unplanned roof fall occurred in the #4 entry, left return of old 3 belt. Fall measured 80 ft long, 19 ft wide and 8 1/2 ft high. This area is worked out. No equipment was involved, no personal injuries reported. Ventilation & travelways were not affected. All approaches have been breakered off.

An unplanned roof fall occured at # 1 Section # 5 Entry near Spad # 863. Ventilation and passage of persons were not affected. No equipment was involved and no personal injuries reported. Fall measured 30' in length, 20' width and 4' high. All approached have been breakered off and will be rehabilitated.

An unplanned roof fall occurred on #1 section #6 entry at spad #17963. Fall measured 45' length, 18' width and 6' height. Fall was inby section dumping point and covered up the Fletcher roof bolting machine, bolter was removed and all entrances have been breakered off.

An unplanned roof fall occurred at parallel mains #3 Entry 1 crosscut inby spad #673. No injuries reported and no equipment involved. Fall measured 200' length, 18' width and 4' to 5' height. Fall was in the intake escapeway. Production was stopped unti escapeway was made passable. Area has been rehabilitated.

Accident type, without injuries

At about 8:50 am a 930E rock truck caught on fire. MSHA was notified of the fire at 9:00am. Local fire departments responded to the fire. The fire was extingushed at 2:30pm. The 930E rock truck received extensive fire damage.The driver of the truck was taken to a local hospital to be checked out. He was released with no injuries. The driver returned to work on 8-9-2007

An unplanned roof fall occurred at the glory hole mains in the #4 entry at spad #3191. No injuries reported. No equipment involved. Ventilation and passage of person not affected. Fall measured 100' x 19' x 2' to 10'. All entrances to fall have being breakned off. Areaa will be rehabilitated. Fall was 12 ft fromt he active face. MSHA and slate were notified immediatetely.

An unplanned roof fall occurred on the #1 section in entry #4 at spad #4367. No injuries reported. No equipment involved ventilation and passage of persons not affected. Fall measured 90' x 19' x 4'-6'. All entrances to fall have been breakened off. Area will not be relbilitated. Fell was 6 roof from the active face. MSHA and slate were notifed immediatley.

EE was fueling up a welder when gasoline splashed on the hot manifold and ignighted the welder which was mounted on a truck. The welder and a 1989 Ford 350 4X4 pickup burned to the ground.

Unplanned roof fall occurred on #5 Belt 1 break inby spad #12186. No injuries. No equip. involved. Ventilation not affected. Passage of persons not affected. Fall measured 6' high & extended approx. 1 break long to belt line. All entrances to fall have been breakered off. Bolted with 48"" resin. Area of fall will not be rehabilitated. MSHA & State were notified immediately.

An unplanned roof fall occurred on the #6A Belt 1 break outby spad #4223. Approx. 420' from the working face. Fall measured 80'x20'x10'. No injuries. Equipment was involved. Ventilation not affected. Passage of persons was affected. Bolted with 48"" glue bolts. MSHA & State were notified immediately. Area has been rehabilitated.

An unplanned roof fall occurred on the #1 section in the #4 entry at spad #17570. No injuries. No equipment involved. Ventilation & passage of persons not affected. Fall measured 50'x20'x10'. All entrances to fall have been breakered off. Area of fall will not be rehabilitated. MSHA & State were notified immediately. Federal inspector investigated the fall on 3/13/07.

an unplanned roof fall occurred in the #7 entry on the #1 section at spad #17553 on the active section. The fall measured 140' length x 19' width x 5' to 6' height. No personal injuries reported. Ventilation was affected. Passage of persons was not affected. All entries to fall have been breakered off. No equipment involved.

An unplanned roof fall occurred at approx 1:00am on the #1 section in the #5 entry which is approx 400' inby spad #2437. The fall measured 20' long, 20' wide and 9' tall. No equipment was involved. Ventilation was not affected. No injuries were reported. Passage of persons was affected. All entrances to fall have been breakered off and fall will not be rehabilitated.

An unplanned roof fall occurred on the #1 section in the #3 entry at spad #5044 approximately 180 feet from the active face. No injuries were reported, no equipment was involved. Ventilation and passage of persons were affected. Fall was measured at 114' length x 20' width x 10' heighth. Fall will be rehabilitated.

Struck against a moving object

Due to continued pain resulting from a mantrip accident that occurred on 12/03/2007, EE went back to the doctor on 03/21/2008 and was diagnosed with two ruptured discs C-5 & C-6. He is currently off attending therapy 3 days a week and awaiting approval for surgery.

Employee stated that while lowering the canopy on the roof bolter he was not paying attention until the canopy struck him in the head. He was diagnosed with post-concussion syndrome.

Over-exertion (Not Elsewhere Classified)

Member was removing a rubber belt guard off a continuous miner and he felt a sharp pain in his groin area. Member was diagnosed with a hernia and is scheduled for surgery to repair hernia.

Fall to the walkway or working surface

Employee was walking across parking lot and tripped on a crib block. Employee fell to ground landing on his right shoulder. Employee continued to work until 11/6/2007 when his Dr. took him off work.

Struck against a moving object

Employee's injury is the result of continuing pain caused by a mantrip accident that occurred on July 27, 2007. Due to increased pain he required surgery to repair tendon in his left shoulder. He was off from 3-4-2008 to 3-24-2008.

Driver braked on flat ground at #17 Marker. Started out in 1st gear, shift to 2nd, as he was breaking over crown to drop off never heard EE brake hit or engine rev up. Passenger asked if he was in gear, & trainee grabbed shifter & wiggled it. It was in neutral, another EE told Inj EE to put it in ditch.

Caught in, under or between a moving and a stationary object

Member was working with the outby crew to lead 5'belt from the roll into a sled that was on a longwall car. When the pulled the car, member stepped up onto the car & proceeded to ride the car through the Aframe belt stairs. His left foot & leg were caught between sled & aframe.

Member was changing a scoop tire when the tire fell and hit him in the chest and arm. Member continued to work until May 4, 2007 at which time he was examined by a chiropractor who stated that he suspected some disc damage and has him taken off work until the diagnosis is continued; a MRI is scheduled.

Employee was waiting for crew to bring in a set of doors to install. He heard motor approaching. As he got up, he stepped on a piece of cinderblock and twisted his ankle.

Fall onto or against objects

Employee was checking engine oil on rock truck. He was stepping back, tripped and fell backwards into the trucks handrails striking his shoulder and neck. The employee was not taken off work until 3-14-2007.

Driver stated he got into berm and went into ditch while driving the coal truck.

Cleaning tailgate area. Right index finger smashed.

Fall from machine

Employee was walking down steps on an 785 rock truck. He slipped coming down the steps hurting his left shoulder. Employee continued to work until April 26th when he was taken off work.

Dropping continuous miner cable down from hangers when he felt a pull in his back.

Over-exertion in lifting objects

Employee was trying to raise hood on a portal bus. The hood latch broke and the hood fell sideways causing the employee to feel a sharp pain in his lower back. The accident was discovered in an internal audit as not being reported.

He was stepping up into the bucket approx 1' off the ground with his left leg when his right foot slipped. All his weight shifted over to his left leg as he twisted.

Employee was walking on the headgate 2 section outby when he stepped in a hole and twisted his right ankle. He was diagnosed with a sprained ankle.

Struck by falling object

A belly pan was being installed after work was completed on a hydraualic line. The pan was being lifted in place by a cable. Once the pan had been lifted, the mechanic moved into place to install the bolts that would permanently hold the pan in place. As this was done, the cable failed and the mechanic was struck by the pan.

Employee was removing clamp on bin elbow. Hand came into contact with sharp piece of metal causing a cut to the index finger.

Employee was drilling into a 2""x4"" piece of lumber when the drill bit hung up in a knot in the wood causing the drill handle to spin, catching his hand in a pinch point.

Employee was stemming holes when he felt back pain. Went to local hospital, was examined and released with no restrictions.

Employee was in the process of cutting a piece of cable when the knife slipped and cut his left index finger.

Member was cutting belt at the tailpiece when knife came out of belt cutting his right knee. About 1"" cut resulted in 3 stitches.

Fall to the walkway or working surface

Member was lifting a drill pot on a flat car when he slipped and fell striking his left hand on the ground resulting in a small fracture to his left wrist.

Struck against stationary object

Member was hanging fire hose over discharge roller, when he cut his little finger on his right hand, on a guard that covers shaft. Member received 5 stitches on little finger on his right hand.

Using a chisel to spread clamp to remove pin. Hit pin with hammer and chisel flew out, hitting EE on the forehead.

Struck by falling object

Member was in the process of using a slate bar to scale the roof, when as he was pulling a piece of loose roof beside the roof bolter, a piece of top fell and struck his right forearm causing a laceration which needed 12 stitches to close.

Caught in, under or between a moving and a stationary object

Member was pushing a scoop tire by hand to be put on scoop. The tire fell over and pinched his right pinky finger in between the scoop tire and the fender of the scoop, causing him to receive a small fracture to the tip of his finger.

Struck against a moving object

Member was operating a 15 ton motor pulling shield cars outside, when he lost control of the motor & wrecked causing a laceration to his head which needed 5 staples to close.

Struck against stationary object

Was fire bossing the section when, as he was traveling across the section, he struck a roof bolt plate with his left shoulder causing a laceration

Employee felt pain in elbow after day of stemming holes. Worked the next day, pain grew worse. Took 8/10/07 off. Initial Dr. visit was on 8/14/07. Diagnosed as ""tennis elbow"".

Employee received an annual hearing test as part of the conservation program in use at out mine site locations, at which time it was noted that he has had a recordable change in the form of moderate hearing loss for high pitched sounds in both ears. He was re-tested within the 30 day MSHA allowable period and moderate loss was confirmed.

Over-exertion in lifting objects

Employee stated he was attempting to lift one side of 3 top rollers banded toogether, when he felt pain in his lower back, resuting in a strained lumbar muscle.
