On a delivery, EE pulled up to a suspicious vehicle, got out of the vehicle, approached the two men regarding their suspicious behavior, got phone out of the truck, while on the phone writing the plate numbers down, the two men jumped in the vehicle & put the vehicle in reverse striking EE, he fell to the ground hurting his back, the two men drove off.
Attempting to operate loaded truck on an incline when it overturned. This occurred at night. EE had seatbelt fastened and received bruising from belt. Due to size of drive the past was removed from the truck so ee could exit from vehicle. When upright truck started and no damage was done & fan on motor. Drive line was damaged and windshield broke, driver side door also damaged.
Operating a dozer; in reverse and ran over a rock, jarred employee causing strain in lower back.
Traveling down a snow/ice covered road, truck that he was driving spun around, got out to look at the road, slipped on ice, lost footing, fell on back.
Employee was walking up a set of steps to access a coal sampling system. As he walked up the first few steps he found that the steps was unsecured and was shaking as he took his first few steps up them. He lost his balance falling forward and fell on his left hand breaking a small bone in his left hand. Employee will need surgery.
Installing hammers on crusher. Hammer slipped hitting right pinky finger. Required 12 stitches.
