Accident type, without injuries

A fire occurred on a vehicle operated by Austin Powder Appalachia while off-loading its cargo of ammonium nitrate. Efforts to extinguish the fire began immediately. There were no injuries as a result of the fire. The fire did not involve the cargo and there was no release of cargo.

Fall onto or against objects

While operating dozer, noticed fuel cap missing and he exited dozer to put back on and fell/slipped on steps.

Helping the mechanic install a new cutting edge on a bulldozer, when the mechanics grinder wheel broke off, striking ee knee.

While working and welding on HWM head, he used a hammer causing piece of metal to strike him in the right eye.

Fall from machine

Employee was leaving dozer cab for a break, slip and fell between tracks and frame hurting left leg/side

Contact with hot objects or substances

while employee was welding, welding helmet was changing dark due to night fall, he changed to safety glasses to finish. A piece of hot slag hit glasses, dripped onto right eyelid, burning lid and eye

Struck against a moving object

While digging solid rock with bucket, the back of loader was raised up, during that time bucket slipped, dropping cab back down suddenly, jarring operator.

Stepped on uneven ground and twisted right leg/knee.

large rock fell out of loader into rock truck, causing jarring to employee and instant back pain

Driver was attempting to drive his truck down a hill, and although it had just been graded the truck lost its traction and starting turning until it turned over.

While using fire extinguisher, employee began coughing from fumes and lost balance causing him to fall down on shoulder.

Pulling nose cone off D9T tractor, lift jack came out of ball on blade, struck right knee and pinned right leg against the back of the blade - resulting in a knee contusion.

Hauling rock to dump area, hit a large mud hole, jarring body and causing pain in neck and back area

Got off of loader to walk around front of loader bucket, tripped over bucket tooth falling flat and landing on wrist

Installing a bearing on a vertical auger. The port-a-power slipped and a piece of metal hit him over eye causing laceration.

Opened door to leave dozer at end shift, while parked on slight down hill the door didn't latch when he pushed open and it started to close back on him, and latch hit his forehead.

He was using air gun to tighten a bolt on miner, when air hose blew off and struck him above left eye

Struck against stationary object

While stepping off HWM, he was struck in the tooth with a hose that was laying at the base of the miner, instantly chipping tooth
