On a delivery, EE pulled up to a suspicious vehicle, got out of the vehicle, approached the two men regarding their suspicious behavior, got phone out of the truck, while on the phone writing the plate numbers down, the two men jumped in the vehicle & put the vehicle in reverse striking EE, he fell to the ground hurting his back, the two men drove off.

EE was climbing off of loader when the weld on the right side bottom ladder broke loose causing him to fall and injure his right ankle/foot.

Employee was getting out of foreman's truck when he tripped on a flashlight wire and fell on his left side on ground straining his left leg.

Employee was working on hoist chain of drill when the socket slipped striking him on the lower lip causing a laceration which required 3 stitches.
