Fall to the walkway or working surface

Employee was assembling highwall miner-using sledgehammer & slate bar. Was prying with slate bar to align bolt holes on structure of 2nd deck, bar slipped & he fell on deck onto his behind.

While shoveling coal on 1/19/2013 felt pain in lower back, then on 1/21/2013 shoveling coal again and pain severly increased

-became reportable after the 3/12/14 office visit - The drill ran over a rock slamming it to the ground jarring back.
