Employee was assembling highwall miner-using sledgehammer & slate bar. Was prying with slate bar to align bolt holes on structure of 2nd deck, bar slipped & he fell on deck onto his behind.

Installing D11 Trans & Bevel gear using a prybar & strained lower back

Laying on back knocking end pins on machine swinging 15 lb sledge hammer pain in abdominal area

Employee was in loader to move coal, lit a cigarette inside cab and a burst of flames burnt arms, previously was cleaning loader

Returning to office, heavy fog resulted in bad visibility, truck left the roadway & struck a rock.

after coming out of office holding pre-shift books, slipped and fell on back landing on steps of office porch

while putting a miner back together, his foot slipped and he fell down slippery conditions

While climbing up forklift, foot slipped, causing to hit leg and then falling backwards.

Opening a gate valve on an overhead bin, strain to abdomen

A rock rolled over striking side of bed he was operating, jarring him and causing neck pain.

Climbing off grease truck and twisted ankle.

Employee was stepping off Rock truck when he slipped missed a step, he fell and landed on his knees

Was climbing down the steps of dozer and slipped. Leg was caught in stairs and strained knee while striking head on steps.

While shoveling coal on 1/19/2013 felt pain in lower back, then on 1/21/2013 shoveling coal again and pain severly increased

-became reportable after the 3/12/14 office visit - The drill ran over a rock slamming it to the ground jarring back.

Replacing radiator hose on 349E excavator in the rain. Struck left hand on hose clamp causing laceration requiring 3 stitches.

Possible middle ear dysfunction (HEARING LOSS) Audiology referred him to ENT
