Employee was shoveling when he strained his right wrist. This was orginally reported back on 8/31/05 as a near miss but waited till the 10/20/05 to request to go see a doctor. This is when the treating physician gave the employee light duty restrictions.

Employee injured by falling stone from overflow of surge bin while installing chute above 4 1/4 chute. (Belt was operating. Multiple stones fell, but EE was not buried.)
EE was pulling a stuck vehicle free from mud and the factory pull hook broke free from the truck which was stuck and the pin struck the 980G loader through the rear window hitting the loader seat from behind, which caused an injury to his right shoulder.
Employee was working on the anvil ring on the VSI crusher, when as the ring was being lowered, his left hand was caught between the ring and the inside edge of the crusher, causing lacerations to his hand.
Employee was climbing a portable work ladder to work on a 992 loader and the weld at the bottom side of the ladder broke, causing the employee to fall with the collapsed ladder, landing on his knees.
EE was changing a Canica crusher feed plate liner, when the liner slipped down and struck the EE in the inside of the left shin.
Employee was performing maintenance on dredge ladder pump. Employee slipped and cut right shin on crossmember of dredge ladder. Injury required 9 stitches.
Employee alleges that she was walking down flight of stairs from Scalehouse and slipped and fell on her backside. She complained of pain to her lower back and was taken to the doctor for treatment.
Employee cut right index finger while removing zip tie.
Employee was walking by the plant shop towards his truck. He tripped and fell on his back against a steel sand tank screw that was placed in that area for fabrication.
Employee was moving a portable pressure cleaner from the wash rack area for repair and while taking it out, it tilted back, he tried to catch it, causing cuts to 3 of his left fingers from a small caution sign mounted on the equipment.
Employee was working near the primary feeder, when he slipped and felt a pain in his back.
Employee was washing out the primary pan feeder in a kneeling position. When he finished the task, he raised up; hitting the top of his left shoulder, striking an I-beam.
Employee was attempting to move an old railroad tie from a stack of old used ones. He got his right hand caught between a rail tie and a section of old rail, when the railroad tie turned in his hands.
Employee was working on a Gleason cable retraction reel when a spring released trapping his middle finger on his left hand.
Employee strained back while lifting scrap metal.
Employee was clearing a plug on the FK Pump for #3 finish mill. He had cleaned out the pump and screw and was working on the flapper when cement that was in the line to the silo came back down the pipe and hit him. It knocked off his hat and glasses and got into both eyes. He wasn't wearing goggles as required.
Employee was washing the concrete floor of the primary crusher area, when he slipped on the wet concrete and fell to the floor, injuring his knee.
Employee was walking up the stairs to his work site when he stepped on a rock and slipped down onto his knee causing a strain requiring physical therapy and medication as treatment.
EE WAS JARRED INSIDE CAB WHEN A BOULDER ROLLED OFF MUCK PILE AND STRUCK LOADER.
Employee was washing down under the secondary plant screening tower with a water hose, when he lost his footing on the ground and fell.
Employee was walking towards his truck in the plant, when he stepped on a rock and twisted his ankle.
EE WAS UNCLOGING THE SCREW FOR # FINISH MILL DUST COLLECTOR WHEN EE GOT CAUSTIC DUST IN LEFT EYE
Employee slipped off the last rung of a ladder and bumped his right knee on the catwalk surface of a conveyor on 2-16-05: (Employee requested medical attention for the first aid incident on 2/23/05, at which point he was taken to workers compensation.)
Employee fractured his right ring finger while installing a motorgrader blade.
Employee got finger caught between a belt and a shieve (fan). Finger was injured while employee took eyes away from hands direction causing the finger to get caught.
Employee was removing door from bucket elevator. While removing the door, employee sprained his right arm. There was material buildup around the door preventing easy removal.
Employee slipped and hit his face on a rock while trying to move a cable for the dragline after the hurricane.
Employee was changing a bearing on the stedman with a portapower jack and while pushing off the bearing the ram popped out and hit him in the jaw area. NOTE: (Employee did not seek any treatment at the time due to mild pain and did not seek medical treatment until 8-29-2005: and was then taken to workers Compensation for treatment)
Employee was opening rail car doors at the loadout area to washout the rail cars and as he was opening the door with a 5 foot bar he slipped and fell backwards. He felt a pain in his left knee. (Employee quit that same day and did not seek treatment for the injury until August 4th, 2005 at which time he was taken to workers comp.)
The employee was taking a survey point on the excavated material Pile on Lake 6 north.As he started to descend the pile, getting towards the bottom, a rock 10"" to 12"" in diameter rolled down the pile and struck him in the back of the legs, causing a bruise and an abrasion on the calves of his legs.
Employee was pulling a water hose to wash down the area, when the hose twisted in his hands, catching the employees right thumb, with the hose clamp, causing a cut on his thumb. (The employee did not seek medical treatment until the 26th of July, at which time he was taken to workers Compensation)
Moving a cable tree when the chain used to move the tree slipped and pinched the employee's finger.
Employee was pulling power cable with a cable puller for the 7820 dragline, when the puller slipped off the cable, causing him to fall backwards, straining his shoulder.
EMPLOYEE, WHILE REMOVING GUARD FOR MAINTENANCE STRUCK CHIN
Employee was changing a broken blow bar in the Humbolt Primary crusher and a small sliver of the blow bar struck him in the left leg.
EE was attempting to tighten a bolt on the drag friction with a hand wrench. The wrench was struck by a drum bolt causing EE's ring finger on L. hand to become pinched between the wrench and drag friction. A fracture to the last digit of the ring finger on the L. hand was realized.
Employee was changing friction brake blocks on the 480W electric Dragline and got some type of dust in his eyes.
