Employee was dismounting from the left shoe of the dragline, while the machine was stopped, by using the 4-step ladder of this side of the dragline. He lost his grip from the last step (approx. 3 feet high), falling backward and landing on his right elbow.

After changing a phase monitor on a electrical cabinet and engergizing to adjust voltage, an arc flash occurred and injured his hands.
Cut back of hand with utility knife while cutting old conveyor belt.
A rock fell from loader bucket and hit his legs.
Tightening head on kelly bar on drill with wrench, wrench slipped and employee moved akwardly and felt pain in lower back.
Employee backed into a lit cutting torch and burned the top portion of his right calf. He was taken to the doctor the following day and diagnosed with 1st and second degree burns.
A non-chargable incident occured at the end of the shift when the two dragline operators turned down the machine. One of the operators told his partner he believed he was having a heart attack. His partner called the EMS 911 and took him to the main gate. Five minutes later the ambulance arrived and took the employee to the hospital where he passed away about one hour later.
