Over-exertion in lifting objects

Employee was lining hopper with plate for wear purposes. He lifted plate overhead and pulled a ligament in his arm.

Employee was lifting a 15 lb motor from its mount and strained his back. Employee has never missed work for more than a day or two at a time; only medical.

EE ENTERED THE WORK AREA (COOLER) DIRECTLY TO START WORKING WITHOUT PREVIOUSLY CLEANING THE AREA OF THE LIME DUST. HE STARTED WORKING WITH HIS P.P.E. (HARDHAT, SAFETY GLASSES, BOOTS AND GLOVES) WHEN HIS HAND BRUSHED UP AGAINST AN ACCUMULATION OF DUST WHICH COVERED HIS FACE AND ENTERED INTO BOTH EYES.

EE WAS PERFORMING ROUTINE MAINTENANCE IN ALL AREAS OF THE PLANT. AT APPROX. 11:00 AM, THE AREA BEHIND HIS LEFT KNEE BEGAN STINGING. UPON INSPECTION, EE NOTICED THE AREA WAS SWELLING AND THROBBING AS WELL AS BEING RED AND IRRITATED. HE PRESUMED A SPIDER HAD BITTEN HIM. EE CONTINUED TO WORK. AS THE DAY WENT ON, THE SWELLING & PAIN INCREASED & HE WENT TO THE DOCTOR FOR TREATMENT.

Fall onto or against objects

Employee slipped and fell hitting his leg inside 480W Electric Dragline while changing hoist cable. (Note: Employee did not seek medical attention until 7/19/2004)

OPERATOR WAS RAISING UP THE BOOM. HE GRABBED THE MASS PIPE AND THE BOOM STABILIZER CRUSHED HIS FINGERS.

Employee was welding inside the welding shop and his foot became entangled in the welding lead which caused him to fall and hit his shoulder on the floor. The ee did not seek medical treatment for the injury until Nov 24, 2004.

Fall from machine

Employee was working in the dragline. He was climbing down the shoe ladder and lost his footing falling 4-5 feet to the ground. Landed on his feet, but felt a strain to his lower back. Working restricted duty and receiving medication and physical therapy.

Employee bumped his knee on the machine, while making repairs on a 926 loader. NOTE: (EE sought medical treatment on January 17th, 2005; prior to this date, it was a first aid log entry)

EE was changing a scraper blade on the belt scraper for the plant blending belt. While attempting to remove the shaft that holds the scraper, his hand was pinched between the shaft & the chute above the shaft. He suffered a small cut & bruising from the accident.

Ee was sitting on the deck of the dredge welding some pipe. While sitting, he turned to pick up an 8"" piece of pipe to weld. He felt a muscle pull in his back but did not feel it bothered him. He reported to supervisor & noted on First Aid form. Two days later, the pain had increased & he felt he needed treatment for the pulled muscle.

EMPLOYEE WAS GRINDING A FLANGE WITH AN 11000 RPM ELECTRIC GRINDER. HE WAS PUTTING THE GRINDER DOWN WHEN IT BUMPED HIS RIGHT LEG JUST ABOVE HIS KNEE. IT TOOK SIX STITCHES TO CLOSE THE WOUND.

THE EE WAS EXITING HER PICKUP AT THE 4600 DRAGLINE WHERE SHE WORKS, WHEN SHE WENT TO CLOSE THE DOOR, SHE REALIZED THE SEATBELT IN THE WAY AND WENT TO GRAB IT, WHICH CAUSED HER TO CATCH HER RIGHT HAND IN THE DOOR AS IT CLOSED.

Employee moved a 70 lb. aggregate sample from a pickup bed to a roller conveyor and felt a sharp pain in his back.

EMPLOYEE WAS STRUCK BY FALLING MATERIAL FROM A HIGHWALL.

The employee strained his back while attempting to move an aggregate sample bucket stand closer to his vehicle. (Important Note) Employee sought treatment for injury on July 21st ,2004.

EE GOT UNIFORM FROM UNIFORM SHED AND CHANGED INTO UNIFORM PANTS. UNKNOWN TO HIM, A SPIDER WAS IN THE PANT LEG AND BIT HIS UPPER RIGHT THIGH. EE IS DIABETIC AND BITE WOULD NOT HEAL. HE WENT TO THE DR TO HAVE BITE CHECKED. DR GAVE HIM A TETANUS SHOT AND PRESCRIBED ANTIBIOTIC.

EMPLOYEE WAS PASSING METAL PLATE THROUGH SHAKER WHEN A SHARP EDGE CUT THROUGH HIS GLOVE CAUSING A LACERATION TO THE RIGHT PALM WHICH REQUIRED STITCHES.

EMPLOYEE WAS PUNCHING IN TO WORK ON THE TIME CLOCK. SHE WAS STANDING IN THE DOORWAY WITH ONE HAND ON THE DOOR JAM AND THE OTHER HAND HOLDING HER TIME CARD. THE DOOR CLOSED AND SHE DIDN'T MOVE HER HAND IN TIME. THE DOOR SMASHED HER FINGER.
