A PAD OPERATOR WAS ATTEMPTING TO RECONNECT TWO DRIP LINES, WHILE PULLING ON THE ONE LINE IT BROKE FREE AND STRUCK THE OPERATOR IN THE RIGHT EYE.

Employee was smelting mill concentrates and a piece of slag popped out of the melt and landed on the ee's arm burning through his coveralls.
