While changing out an equalizer bar on a D10R Track Dozer, an employee had his hand in a pinch point between the equalizer bar and Dozer frame while he was using a wrench. The employee's left pinky and ring finger were pinched between the frame and equalizer bar resulting in a crushing/amputation injury to these two fingers.

While attempting to break core, employee put his hand on the water swivel and lowered the drill steel and caught his right index finger between the power head and water swivel. Employee sustained a dislocated and laceration injury.
Employee was tripping in pipe and hand was caught pulling up and twisting arm. Injury occurred on May 3, 2011 but employee did not see a physician or receive medical treatment until May 31, 2011.
Employees were lowering a full core tube at a slow speed. Employee was holding the tube about to put it on the rod rack when the wire line sped up causing the employee to lose control of the core tube. While the employee was trying to control the tube he felt a pull in his left shoulder. On 03-24-11 employee stated his arm was hurting and received medical treatment.
As an employee was adjusting pipe in the fusing machine, the loader operator pushed the pipe further into the machine and pinched the employee's left hand middle finger. The EE was wearing gloves at the time of the incident. The EE received 6 stitches and their finger was fractured.
Employee was tripping in and as the plug was being screwed into new rod the plug slipped out of thread, as plug was falling employee grabbed plug and finger was pinched between plug and head slide.
Worker was holding sign when Backhoe operator went to move Backhoe and back of Backhoe swung and hit his finger
Employee was air arcing on a mantle liner, when a piece of molten slag burned through his pant leg and fell into his boot. The slag caused 2nd and 3rd Degree burns to his ankle before he could get his boot off.
An employee fell off of the base of a ""candlestick"" (used to bridge electrical cable over haul roads). (Base is approximately 36"" above ground level. The employee received a laceration to the left side of their head requiring eight sutures.
A mechanic was installing a shank on a dozer ripper. The dozer operator which was standing nearby was struck by a piece of frag metal.
On April 4, 2011, an employee was performing normal job functions and began experiencing pain in the employee's groin area. The EE was transported to the RMGC Clinic and diagnosed with a possible hernia. The EE was released back to work and a follow-up scheduled with a specialist. On May 6, 2011 the employee had surgery to repair the hernia.
