Fall from machine

Dismounting D11 Dozer in the Truck Shop.

Fall to the walkway or working surface

Weather: sunny, haul road ground conditions with medium-sized rock in the road.

Pulled a muscle while walking and bending over on the blast pattern.

Caught in, under or between a moving and a stationary object

Employee was attempting to adjust bolt alignment fingers on bolter using a hammer, while doing so the employees left hand pinky finger got caught between the hammer and the alignment finger causing a laceration. Employee received 7 stitches.

Inhalation of radiations, caustics, toxic and noxious substances

During a tail screen change at the West Dedicated Plant, an employee was potentially exposed to HCN. After the task, the employee reported lightheadedness and nausea, was evaluated at the onsite clinic, and transported to a hospital. No injury or illness was diagnosed.
