Over-exertion (Not Elsewhere Classified)

Operator was helping load a utility truck with 4x12 sheets of sheet rock. (Operator continued to work until 2/13/06) (2007100030-the operator was off again on 10/31/06 Short term Diability. Negotiation have been ongoing between the Workman's Comp. Carrier and the Doctor as to whether or not this injury is related to the initail incident.)

While unhooking a drain hose from a carbon truck, an employee felt what he thought was an eye lash in his eye. The employee rubbed his eye with his hand. The object did not move and had become embedded in the eye.

Employee was walking across the yard towards the batch plant when a large gust of wind came up & blew dust in her eyes. She had help flushing her eyes out with saline solution. The next day she came to work & her eyes were still irritated so she was taken to a Dr. where more material was removed from her eye.

Caught in, under or between a moving and a stationary object

An ore process employee was cleaning a discharge chute with a spader. The spader caught his hand in a pinch point.

Struck by flying object

Wind was blowing extremely hard - when employee got out of haul truck at fuel island & felt something hit her eye.
