4 workers were taking screens out of tower early in the shift. Later, supervisor looked & saw no one so he turned the plant on. Victim was standing on a belt conveyor, working inside the chute & the belt conveyor started. He was pulled out of the chute & conveyed under 2 other chutes located on the same belt conveyor. After the belt was shut down he was found under 3rd chute.
Employee was changing screens and pulled a muscle in his back.
Employee was cutting skirt rubber when the blade slipped and cut his finger
Employee bent over and felt a pain in his back
Employee said he hit a bump in the road and the seat bottomed out. He felt a pain in his back sometime during the day. He did not report it until the next morning.
Employee started to climb in the loader and felt something pop in his back.
