Caught in, under or between running or meshing objects

4 workers were taking screens out of tower early in the shift. Later, supervisor looked & saw no one so he turned the plant on. Victim was standing on a belt conveyor, working inside the chute & the belt conveyor started. He was pulled out of the chute & conveyed under 2 other chutes located on the same belt conveyor. After the belt was shut down he was found under 3rd chute.

Struck against stationary object

Employee was preforming routine duties by picking up deliveries, when they lifted a package up hitting their head on the garage door. While the employee was wearing a hard hat at the time, they felt a pain in their neck as a result. Employee is currently resting at home after medical attention, expected to return next week.

Employee was working on screen doing a product screen change. EE was working with another employee. EE was in the hopper under the screen removing hardware for center hold down strip when a rock fell through the screen hitting them in the back of the neck.

Cleaning up a weld with a chipping hammer a piece of slag flew up under the helmet and caused irritation to the eye.

Misjudged the height of the step into the time clock room, lost balance and fell.

Attaching a cylinder to the blade of a dozer ---when the pry bar slipped and the cylinder smashed the tip of the Emp left middle finger --- requiring stitches.

While wrenching on a cylinder on a piece of equipment the wrenches pressed together and cut into the nail and tip of the mechanic's rt ring finger requiring stitches.

Struck by flying object

While greasing the cylinder on the Dozer the cylinder head did not appear to be getting the grease. The employee looked over the cylinder head grease fitting and it popped off and struck EE in the right eye

Employee was getting out of vehicle. There was a slick spot. Employee slipped and fell causing injury to left hip.

Struck by flying object

Emp was hitting shoe on a blow bar and hit a hard surface, a piece of steel struck and cut EE's finger. EE got 1st aid and continued to work. Emp worked until 9-28-2020 when they found a piece of steel in EE's finger and had it removed. EE then went on RD as of 9-29-2020.

While getting off the loader, the employee, slipped on step (did not fall) and strained back. Employee went back for a follow up on 11/22/2019 and P.T. was recommended... on 11/25/2019 Employee signed up for P.T. --- making this injury reportable.

Emp suffered a burn to left hand while heating up an elbow. EE used bare hand to re position it.

Struck by falling object

A rock (football size) rolled down and off a catwalk, striking the ground person in the back as EE was bending over shoveling. EE went to the clinic and was put on restricted duty with a bruise to the lower left side of EE's back.

Contractor Employee was working on tracks of excavator and had to remove built up dry dirt to get to the bolts. As they were scrapping the dried dirt, a small sliver of metal became embedded into the left forearm

Fall from ladders

Employee was traveling down a ladder, tripped and struck right knee on block ... reported injury on 7-28-17. Emp went to Clinic and was put on restrictions. Emp doing regular job (missing no time) but working with-in restrictions.

Over-exertion in lifting objects

Employee was inside a head-box cleaning it out. As employee went to exit the box employee felt a strain in employee's back.

Fall onto or against objects

Employee injured knee when employee slipped climbing up an excavator to fuel it. This slip / injury happened on 6/2/17. Employee continued to work (after receiving 1st Aid) and worked without issue until employee woke-up on July 5th in pain. On July 5th employee had knee drained and was back to full duty on the 10th of July.

Over-exertion in lifting objects

Employee was lifting a 50 pound 20 foot long piece of angle iron and felt a muscle pull in rib cage. Employee went to clinic and returned to work with lifting restrictions.

Fall from machine

Employee was climbing on to the 1st step of haul truck and went to place wheel chalk in place....ee slip and fell 18 inches and fractured left wrist

Inspecting the Dozer, Employee was dismounting the Dozer and slipped, spraining EE's left knee. Employee reported the sprain to management immediately. Employee and management decided in house 1st aid was enough. It was later decided (on 3-25-16) to seek professional help.

Employee was standing on unstable ground...lost balance and grabbed for near by pipe for support. Employee's ring caught the angle iron that was supporting the pipe and almost tour his ring finger off.

Employee slipped and fell to his left knee while walking down a dirt road. The road had a few ruts in it due to rain. Employee stepped next to a rut that gave way and caused him to lose his balance and fall to his knee.

Employee was working with a partner taking out bolts on the primary crusher wear bar. one employee was working the ratchet while the other supported the socket. As the socket started to slip off the bolt the employee who was injured reached to support the socket when the socket slipped off the bolt and the employee was struck by the ratchet... injuring his rt thumb.
