Struck against stationary object

Employee was repairing a tear in a conveyor belt. While pushing down on the belt clip a sharp edge, his rt index finger was cut

Fall to the walkway or working surface

Employee slipped and fell to his left knee while walking down a dirt road. The road had a few ruts in it due to rain. Employee stepped next to a rut that gave way and caused him to lose his balance and fall to his knee.

Employee was working with a partner taking out bolts on the primary crusher wear bar. one employee was working the ratchet while the other supported the socket. As the socket started to slip off the bolt the employee who was injured reached to support the socket when the socket slipped off the bolt and the employee was struck by the ratchet... injuring his rt thumb.

Employee failed to use the proper tool when lining up 2 pipes and caught his finger in the gap, cutting the tip of the left index finger.

Employee was using a knife to open up a package of soap stone and cut left hand pinky finger... requiring 4 stitches.
