Caught in, under or between a moving and a stationary object

EE was in the process of unloading railcars. EE then left the sight of the other 2 employees in EE's location. They then heard EE screaming for help and found EE lying adjacent to the train. Both lower legs had been amputated.

Fall onto or against objects

Operator was unloading railcars when operator missteped between surfaces falling into gap causing abrasions to left leg and thigh and also straining left shoulder.

Miner was prying on the track adjuster of a D9 dozer when the pry bar broke hitting EE in the forehead causing a laceration.
