Struck against a moving object

Vega employee was driving a 10 Wheel dump truck following a TIC employee up a hill at Newmont. It was slippery and the truck lost traction and slid down on its side. Vega employee was taken by Newmont ambulance to Northern Nevada Regional Hospital.

Struck against stationary object

Employee was collecting ore control sample and stepped on a rock and lost footing and twisted his back and he suffered a lumbar strain.

Fall onto or against objects

While getting on the crew bus employee tripped and fell. His right hand contacted the bottom step of the bus. This contact resulted in a fractured metacarpal bone behind his pinky finger. He did not seek medical attention until 11/16/2010.

Caught in, under or between a moving and a stationary object

Driller was adding drill rod. Top of rod contacted sheav wheel and dropped back on hand. Finger pinched against feed frame of drill.

Fall to the walkway or working surface

Checked on employee when returning to office he slipped on ice.

Absorption of radiations, caustics, toxic and noxious substances

While unplugging a shotcrete hose, cement sprayed in his face. He received medical treatment 7/28/10.
