Struck against a moving object

While the employees where traveling out of the mine the floor mat wadded up in such a way that it caused the throttle to stick and maintain speed when braking was necessary. The vehicle collided with the rib and resulted in a shoulder injury.

Over-exertion (Not Elsewhere Classified)

Employee was hanging electrical cable from a man basket off of the forks on a forklift. Employee bent over to get under a 42"" ventilation bag and twisted employee's left knee.

Struck by falling object

EE was struck by a rock from the face while installing color pipe in the lifters. EE was bent over and was struck in the back of the head and neck. EE received 9 stiches on the top of head
