Ran over a piece of steel with UG haul truck and it flipped up and hit him in the mouth.

Contact with hot objects or substances

While changing a broken fanbelt on the Toro loader, Company #23, the top hose was loosened to relieve the cooling system pressure. When the hose was removed there was enough presure remaining in the system that the coolant sprayed out on employee, burning his arms and chest.

Removing slide on rock bolter with impact wrench. Retainer on slide spun hitting left index finger cut it.
