Struck by falling object

While bolting in the face the left rib failed back from the face and buried the operator.

Caught in, under or between a moving and a stationary object

While remote mucking, the mucker got too close and pinched him between the rib and the mucker.

Over-exertion in lifting objects

When moving a truck ramp him pulled his arm or shoulder resulting in restricted duty.

Struck against stationary object

While stepping off tractor he twisted ankle on a rock.

Struck by flying object

While bending a piece of 11"" x 2"" x 1/4"" steel with sledge hammer the welds that held the material to the table broke loose and hit the employee in the chin.

Caught in, under or between a moving and a stationary object

While installing a new rod on the Cubex Drill the employee placed his hand on the bottom of the rod. The rod slipped and caught his hand between the two rods. The injury required one suture.

The employee was tightening a bolt with an impact wrench when the pin holding the socket came loose and hit his hand requiring two sutures.
