IE WAS HOISTING A DISCHARGE PIPE WHEN THE PAD EYE BROKE LOOSE STRIKING THE FACE.

Miner was hauling gob from the prep plant when EE suddenly noticed a coal hauler had slowed in front of EE. When miner made contact with coal hauler EE swerved to the ditch and turned over on side.

IE WAS CLIMBING UP ON DOZER AND GRABBED THE HANDRAIL AS EE STEPPED UP FOOT SLIPPED OFF THE STEP PULLING ON EE'S ARM.

Miner was lowering the tracks down to remove the slack from the tracks and was struck in the arm with a foreign object.

IE WAS CLIMBING UP ON EQUIPMENT AND SLIPPED ON STEP AND FELT A STRAIN IN RIGHT HIP.

IE was stepping into a powder magazine and slipped on a muddy floor striking shin on the floor and scraping back on the door jamb.
