Caught in, under or between a moving and a stationary object

While moving a head roller shaft, IE attempted to set the shaft on cribbing, but as the shaft was being set down, it shifted which pinched the IE left hand between the shaft and a sheave that was on the floor beneath the shaft. This resulted in two fingers of the left hand receiving fractures and lacerations that required sutures to close.

Struck against stationary object

Employee was working with the blasting crew. They were on the bench waiting to move to the next hole to load. When it was time to move employee stepped back and stepped on a frozen rock, which rolled an ankle. Later after x-rays it was determined the ankle was broke.

IE WAS CHECKING FLUID LEVELS ON MACHINE AND THE COMPARTMENT DOOR FELL DOWN SMASHING THE END OF FINGER.

Miner was lowering the tracks down to remove the slack from the tracks and was struck in the arm with a foreign object.

Miner was tightening a hose fitting when wrench slipped causing miner to lunge forward and hit head on hose fitting.

Miner climbed up to retrieve lunch bucket from the cab and when EE went to exit EE missed the step and fell to the ground.

Employee was operating a gob truck, backed up to the berm and began to dump and felt the truck move backwards. EE attempted to pull up but felt the truck still sliding. EE exited the truck cab and jumped to the ground injuring ankle and both heels.

Truck 376 had a front bumper marker light pole that was broken and had a sharp edge on it. Driver got out of truck and came into contact with the broken sharp edge and cut EE's right arm.

Miner was getting off of dozer when EE notices the step was full of dirt. EE stepped off the push arm (approximately 3 feet) to the ground and landed wrong on EE's foot and twisted EE's ankle.

Employee was installing a bearing on the left front strut on end-dump. Employee was hammering a bearing in place with a 10 pound hammer while holding it with his left hand. Employee's hammer become entangled with some hoses and missed the bearing and struck his hand with the hammer.

Laborer was removing the wheel chalks from fuel truck when he felt a sharp pain in his back at the end of day-shift. He returned to work his next scheduled shift and had stiffness in his back. Employee returned home.

Miner was reaching to flip on Master Switch and felt a strain in rib area.

Employee had just pulled his powder truck under the powder bin to reload. The powder truck was parked in a rut requiring him to step a little higher to the first step. The employee then felt pain in his groin area. The employee continued to work the rest of the shift and returned the next day but the pain increased so he went to get it checked.

The miner was helping another miner tighten a bolt on a Dryer Feed Tube with an impact wrench. The impact socket had an adapter connected to it by a 1/4 inch bolt. The miner was holding the socket on the bolt head when his hand came into contact with the spinning socket. The miner received a cut between his two middle fingers requiring 4 sutures

Employee was operating end dump # 1158 and after 2 or 3 loads he said his right shoulder was hurting. Right shoulder strain.

Employee stated he was driving down ramp that had just been watered for dust control. when he got to the bottom when he got to bottom of ramp he need to steer quickly using his left arm. pulled muscle or strain muscle in left shoulder.

The tire tech helper was guiding the 15 ton jack hanging from chain under a R190 Euclid when the boom got stuck on the frame of the truck and broke loose. Causing the jack to pop up and hit the tire tech helper in the forehead and nose.

Steamer was using a ladder to access area which needed to be cleaned. The ladder became unstable. He fell between 5 & 6 feet, striking his arm on fender, cutting forearm which required 9 stitches to close wound. It was a minor wound which caused no loss of work.

EE was climbing on unit # 1230 dozer he slipped and caught himself with his left hand. He said his left arm was sore after. He worked until 7/10/13.

Employee was in parking lot for dozers when he thought he felt a shock. At the time he did think he was hit by lightning. He went home and worked the next day June 2, was off June 3 and 4, back to regular work on June 5 2013.

Employee was hitching water pump (1601) to IT loader (1045). He was cranking the jack up to align the hitch when the jack broke off the pump. The tongue of the pump fell on his right foot. His foot was bruised above his steel toe.

Employee was working on a overflow box and trying to get it to set into place. He was using a chain come-a-long. The come-a-long came loose and the chain hit employee in the chest. (Contusion to the ribs) Employee went to E/R that night. Then worked two days and went back to doctor.

Employee had climbed up the ladder to check fuel level in fuel truck. When he was coming down his foot slipped and he grabbed for the hand rail but missed it. Then he fell to the ground, about 4 feet, he fell on his back. (Bruised lower back)

He was running a tractor pulling a scraper at the Shamrock Mine. He was wearing a seatbelt and not driving at any excessive speed. He crested the ramp and hit a rut. The bounce that it cause made him sprain his back.

employee was changing the oil in water pump #1578 the wrench slipped off the oil pan bolt and his arm hit a piece of metal on the frame. this caused a laceration on his arm. seven stitches.

Employee was repairing Komatsu dozer and was using an 1/2 impact wrench when a wobble sock on the wrench came off and hit him in the mouth. Contusion and broken tooth. He was removing bolts from 475 Komatsu dozer drive shaft.

Employee was making his inspection of his coal truck and when he went around the truck he slipped on the ice and fell to the ground. He injured his left shoulder. Contusion/impingement.

EE was removing a bolt from a belly pan on a D10T dozer. He was cutting the bolt with a torch he had a face shield on and a piece of slag got under the shield and into his eye. EE didn't know it till later on his way home.

EE was hitting the end of a carrier roller shaft that had worked out of the trackframe of the dozer. When he hit it the second time a piece of metal chipped off of the roller and pierced his leg around the back of the knee. He got 4 stitches.

Employee was putting plastic pipe on the forks of the IT14 Loader. One of the forks fell off and landed on his left foot. (contusion left foot)

Employee was digging tracks on dozers, when he was finished he said his shoulder was hurting. He went to Imed for medical treatment. Strained right shoulder.

EE was fueling equipment. He was pulling out the hose on the fuel truck and his back started to hurt. He went to the emergency room and Imed clinic. Back strain.

Employee could not get side door open on 988B loader unit 1040. Employee was pulling on side door. Had trouble getting door open. Lost his grip and hit himself in mouth. Broke one tooth off and chipped the tip of one tooth.

EE was walking in washplant, he walked under the spirals and when he looked up, some fines fell off of a pipe and got in his right eye.

Employee was putting new hydraulic pipe under the body of the Hitachi shovel. He was standing on a short ladder and slipped. He grabbed a hydraulic pipe to keep from falling and hyperextended his left shouler.

EE was working on dozer placing nuts on the bolts when a piece of dirt got into his eye. EE rubbed his eye and got a piece of metal in his eye. EE didn't know if the metal was in the dirt or on his hand. EE was helping put tracks on the dozer

EE was in parts room getting parts and when he came out he felt something in his right eye. EE did not know when he would have got something in his eye. Dirt in eye, it was windy that day.

EE was building up a lift with rock and soil bounced in dozer, back stain. On 3/10/2010, EE said his back was hurting, never missed any work till 10/29/10.

Employee was helping replace a pipe on the second floor of the wash plant. He strained and injured his right shoulder. He has pain in shoulder and intense burning. He went to the doctor on the twenty second.

Employee told his foreman at the end of his shift that he might not be in the next day because his back was hurting. Employee called the office the next day and said he had been to the doctor because he had hurt his back driving his end dump over rough roads.

EE had a problem with his hitachi shovel and was trying to get the ladder down, he jerked on the latch to lower the ladder and injured his back. Frozen mud was keeping the latch from releasing, the mud was removed and ladder lowered.

Employee and mechanic were thawing out an air line when it ruptured. Alcohol in the line flashed and employee was burned on his hand.

Employee was working on #2 belt press. He was using a ratchet chain fall to move a roller back. His hand slipped off the chain fall handle hitting his right hand on the belt press cutting his hand requiring 2 stitches.

EE was under a dozer. He was trying to put a pin back in an equalizer bar which the pin had come out, on the track frame side the equalizer bar dropped about 2 "". EE moved out of the way and hit his head on something under the dozer maybe a bolt 1 1/2"" laceration requiring 8 staples.

EE was climbing up the ladder on the water truck his foot slipped and he fell injuring his right wrist.

Employee was helping remove a large rock that was stuck on the feeder belt. When trying to get the rock in position on the belt it rolled and employee had his hand on belt and rock rolled on his hand causing a contusion to right hand.

Employee was changing a hydro senser in the hydraulic system of a end dump. Employee had bed raised and secured. He had released the pressure from the tank. When he removed the sensor, hot oil sprayed on his hands resulting in a 2nd degree burn to his thumbs and 1st degree burns to his hands. Unit #1155.

Emplloyee was picking up water hose. He was rolling up hose and jerked on the hose and pulled a muscle in his back.

EE had a scratch on his hand and noticed that it had a sore on it. He broke the sore open and thought it was fine. The next day his hand was swollen when he got up. Does not know when he scratched his hand.

EE was getting off of his dozer and did not see the frozen dirt on the push arm and slipped off the arm, striking his left shoulder on blade stabelizer.

Employee was digging tracks on shovel and a rock fell off of the track frame and landed on his foot above the steel toe fracturing his left foot.

Employee was standing on a portable ladder when it slipped and he fell to the ground bruising his shoulder and right leg. Fell about six feet. Employee was taken to ER and released to go back to work on 7/31/08 went back to DR, shoulder was still hurting and EE was put on restricted work.
